The South African Civil Aviation Authority (SACAA) suspended the approval of Lufthansa Technik, one of the aircraft maintenance organisations that maintains Comair's fleet. This came after a four-day audit found four level 1 findings with Lufthansa's quality and safety management systems, posing an immediate risk to aviation safety. While Lufthansa addressed two findings within 24 hours, the SACAA issued a 24-hour precautionary suspension as the other two findings remained unresolved. The SACAA takes its mandate to regulate aviation safety seriously and will enforce regulations without hesitation.

Each Piece of PMV is to be taken out of service on a vehicle-by-vehicle basis across all projects until the following 10-point checks of each piece of plant and equipment (including those of subcontractors and ATC hired plant) have been verified:
1. Vehicle Repair and Maintenance records.
2. Testing and Verification of Reversing Alarms, Beacon Lights, Seat Belts, Rear View Mirrors, Brakes, Hydraulics, Tire Pressure, Tilt and Load Indicators on all equipment (where appropriate) and other safety features to ensure they are functioning.
3. Verification of Equipment by Third Party fitness certification and internal inspection records.
4. Vehicle registration and insurance documents.
5. Verification of Operator Credentials (Third party operator certification, ATC or Supplier training records, driving license).
6. Appropriate PMV training documents (multi lingual) for each type of PMV, to be provided to the logistics team, supervisors (of all types and levels) involved in PMV operations including operators, riggers and flagmen.
7. A Task Risk Assessment for each type of PMV (to be submitted to HSE Department).
8. Operator Pre-use Safety Checklists are being used, on a daily basis.
9. A PMV Inspection & Maintenance Plan, or similar, for all PMV being used at Project (to be submitted to HSE Department)
10. After the vehicle and operator check sheet is signed off then the vehicle can be returned to full service via a progressive, vehicle by vehicle basis via a Return to Work Permit.
Note: All of the above should already have been in place

All Non-Tower Crane Lifting Operations are to be temporarily suspended on a vehicle-by-vehicle basis across all projects until the following 10-point checks (which you should already have in place) of each piece of plant and equipment (Including there of subcontractors and ATC hired plant) have been verified:
• A Lifting Plan or similar for each type of lifting equipment (to be submitted to HSE Department)
• A Risk Assessment for each type of Lifting Equipment (to be submitted to HSE Department).
• Appropriate lifting training documents (multi lingual) for each type of lifting machine, to be provided to the logistics team, supervisors (of all types and levels) involved in lifting including operators, riggers and flagmen.
• Verification that lifting tackles are safe, in good condition, ID tagged with #, date and fit for use.
• Verification of Operator Credentials (Third party operator certification, ATC or Supplier training records, driving license).
• Vehicle registration documents.
• Verification of Equipment by Third Party fitness certifications and internal inspection records.
• Internal Inspection Records (Use operator Pre-use Safety Checklist)
• Vehicle Repair and Maintenance records.
• Inspection & Verification of Tire Pressures, Tilt and Load Indicators on all equipment (where appropriate) to ensure they are functioning.

In aviation sector the Airline operators are earning from the number of routes they are operating.
Larger routes earn more profit comparable to short domestic routes. If an airline is banned to operate to
foreign routes, other carriers can take an opportunity to attract the passengers toward themselves during the
ban period of that operator. The penalty of losing route, passengers and their trust becomes colossal when
considered in terms of revenue and cost. European Union agency for aviation safety assessment has set
standard procedures for inspection of aircraft operator’s safety level and can put ban or limitation on the
operators to operate in their territory. So keeping this fact in mind case study of Pakistan international Airline
is taken, which came under the observation of European safety agency in 2006 and 2010 for degrading safety
level. In this work possible contributing factors that can be a cause to leading of non-conformance are analysed.
Main objective is to target out the main bottleneck through this case study and suggest the solutions to sort out
that problem.
The Department of Homeland Security & Transportation Security Administration have extensive rules that must be complied with when shipping cargo via air freight. Knowledge of the "chain of custody" is to be affirmed in all cases.

CIVIL AVIATION AUTHORITY SUSPENDS LUFTHANSA AIRCRAFT MAINTENANCE
ORGANISATION APPROVAL
Johannesburg – The South African Civil Aviation Authority (SACAA) suspended the approval of
Lufthansa Technik, one of the Aircraft Maintenance Organisations (AMO) that maintains the fleet of
Comair Ltd on Monday 22 March 2022. The suspension follows a four-day audit which started on
15 March 2022. The audit of the both the AMOs maintaining the Comair fleet follows the conclusion
of the airline’s audit by the SACAA following a spate of incidents taking place from February 2022.
During the safety oversight visit Lufthansa Technik’s quality control management system (QC) and
safety management systems (SMS) were subjected to a review to establish legislative compliance
related to reporting, analysis and follow-up of occurrences, and corrective action plans to prevent
recurrence.
The audit resulted in four Level 1 findings raised with the AMO. A Level 1 finding poses an immediate
risk to users of civil aviation services and such findings must be closed immediately. The SACAA
shared the preliminary results of the audit on Sunday with Lufthansa’s accountable manager and his
team and gave the operator twenty-four hours to make representations. The SACAA reviewed the
evidence and corrective action plans submitted by the Operator until Monday 21 March 2022. The
evidence was found to be satisfactory in relation to two of the Level 1 findings. This means that two
other Level 1 findings could not be closed within the stipulated time. It is for this reason that the
SACAA handed a 24-hour precautionary suspension of the privileges of the AMO with effect from
Monday night. The operator is required to submit further evidence to close the gaps identified by the
Regulator within this period failing which the Regulator will indefinitely suspend the approval of the
AMO until such findings are sufficiently closed.
The SACAA found that the AMO had both a quality management system and safety management
system in place however both were not implemented as per the CARs and the requisite manuals.
According to the Civil Aviation Regulations (CARs), the Operator can appeal the decision of the
suspension to the Director in terms of Part 185.04.2.
