Recomendações da OMS sobre cuidados maternos e neonatais para uma experiência pós-natal positiva.
Em consonância com os ODS – Objetivos do Desenvolvimento Sustentável e a Estratégia Global para a Saúde das Mulheres, Crianças e Adolescentes, e aplicando uma abordagem baseada nos direitos humanos, os esforços de cuidados pós-natais devem expandir-se para além da cobertura e da simples sobrevivência, de modo a incluir cuidados de qualidade.
Estas diretrizes visam melhorar a qualidade dos cuidados pós-natais essenciais e de rotina prestados às mulheres e aos recém-nascidos, com o objetivo final de melhorar a saúde e o bem-estar materno e neonatal.
Uma “experiência pós-natal positiva” é um resultado importante para todas as mulheres que dão à luz e para os seus recém-nascidos, estabelecendo as bases para a melhoria da saúde e do bem-estar a curto e longo prazo. Uma experiência pós-natal positiva é definida como aquela em que as mulheres, pessoas que gestam, os recém-nascidos, os casais, os pais, os cuidadores e as famílias recebem informação consistente, garantia e apoio de profissionais de saúde motivados; e onde um sistema de saúde flexível e com recursos reconheça as necessidades das mulheres e dos bebês e respeite o seu contexto cultural.

The Ohio State University Comprehensive Cancer Center, will provide insight into new directions in targeted therapeutic approaches for older adults with mantle cell lymphoma.
STATEMENT OF NEED
Mantle cell lymphoma (MCL) is a rare, aggressive B-cell non-Hodgkin lymphoma (NHL) accounting for 5% to 7% of all lymphomas. Its prognosis ranges from indolent disease that does not require treatment for years to very aggressive disease, which is associated with poor survival (Silkenstedt et al, 2021). Typically, MCL is diagnosed at advanced stage and in older patients who cannot tolerate intensive therapy (NCCN, 2022). Although recent advances have slightly increased remission rates, recurrence and relapse remain very common, leading to a median overall survival between 3 and 6 years (LLS, 2021). Though there are several effective options, progress is still needed towards establishing an accepted frontline approach for MCL (Castellino et al, 2022). Treatment selection and management of MCL are complicated by the heterogeneity of prognosis, advanced age and comorbidities of patients, and lack of an established standard approach for treatment, making it vital that clinicians be familiar with the latest research and advances in this area. In this activity chaired by Michael Wang, MD, Professor in the Department of Lymphoma & Myeloma at MD Anderson Cancer Center, expert faculty will discuss prognostic factors informing treatment, the promising results of recent trials in new therapeutic approaches, and the implications of treatment resistance in therapeutic selection for MCL.

2. Adenomyosis is a lesion that occurs
when the endometrial glands and
interstitium invade and grow into the
myometrium of the uterus. The main
symptoms are increased menstruation,
prolonged periods and progressive
dysmenorrhea, which can lead to
miscarriage or infertility.
3. It occurs mainly in menstruating
mothers aged 30-50, with a prevalence
of 7%-23%, about 15% combined
with endometriosis, and about half
combined with uterine fibroids. It has a
severe impact on the physical and
mental health of patients.
5. When a patient with adenomyosis
becomes pregnant, menstruation stops,
and the patient does not experience
menstrual pain because the estrogen
level decreases during pregnancy. The
ectopic endometrium also stops
growing and shrinks to a certain extent,
providing a certain degree of relief and
control of the adenomyosis condition.
6. The effect of pregnancy in alleviating
adenomyosis is similar to drug therapy.
Pregnancy is more effective than
medication in controlling adenomyosis
because the gestation period is 10
months long.
8. It is not absolute that adenomyosis
cures itself after pregnancy and
childbirth, but the patient does not have
menstruation without symptoms during
pregnancy. However, after delivery, the
menstruation may recover, and the
adenomyosis may recur or even worsen.
9. Therefore, it is recommended that
patients continue to monitor their
condition after childbirth; if they find
any disease recurrence, they should be
promptly admitted to the hospital for
examination and treatment.
Breastfeeding can also be done to
prolong the time without menstruation.
This will better relieve the condition.
11. Generally speaking, natural childbirth
does not cause adenomyosis. Most
cases of adenomyosis are due to
cesarean sections.
During a cesarean section, it is likely to
cause damage to women’s uterine
wall, allowing the blood in the uterine
cavity to contain endometrium and to
enter the myometrium or grow
ectopically in various parts of the pelvis,
thus causing endometriosis.
13. In addition, women with multiple
abortions and curettage may also
damage the uterine wall, causing
ectopic endometrium implantation and
triggering adenomyosis. It is widely
believed that some uterine operations,
such as cesarean section and multiple
abortions, are one of the main factors
that cause adenomyosis. It is also
associated with genetics, endocrine
disorders, viral infections, and
inflammatory diseases.
14. Many treatment methods for
adenomyosis need to be selected
individually according to age,
symptoms, and reproductive
requirements. Most use the treatment
scheme of combining surgery and
drugs. For drug treatment, Fuyan Pill, a
traditional Chinese medicine, can help
eliminate symptoms and restore the
health of your reproductive system.
Severe patients can be treated
surgically.
15. Since the cause of
adenomyosis is unclear
and no effective
preventive measures are
available, the following
aspects need to be
noted.
16. Because repeated abortions, scraping,
etc. may make the endometrium ectopic
and cause adenomyosis. If you don’t
want to have children for the moment,
you should do a good job of
contraception and avoid repeated
abortions or scrapings.
17. Failure to do a good health care job
during menstruation may also allow the
shed endometrium to enter the muscle
layer and induce adenomyosis. To prevent
this disease, you should pay attention to
personal hygiene; take proper rest and do
not do strenuous exercise; control your
emotions and do not sulk all the time;
avoid sexual intercourse during
menstruation.
18. To prevent adenomyosis, do the
prevention and treatment of endometritis.
Once this disease occurs, actively apply
antibiotic treatment to relieve the
condition and reduce the incidence of
adenomyosis.
19. You must consult a doctor when troubled
by irregular menstruation, dysmenorrhea,
and other problems. Early detection and
treatment can prevent endometriosis
hyperplasia, and the incidence of
adenomyosis will also be reduced.
20. You should avoid unnecessary
gynecological examinations during
menstruation; if you must, do not squeeze
the uterus with force.
