This document is a formative assessment test for 5th grade science students. It contains multiple choice questions, matching questions, fill in the blank questions, and short answer questions testing students' knowledge of human anatomy and physiology. Topics covered include the skeletal system, circulatory system, nervous system, senses, muscles, vitamins and minerals, cells, and diseases. Students are asked to identify bones, organs, tissues, nutrients, and pathogens. They must also explain deficiency diseases and the importance of the skeleton. The last question requires drawing and labeling a diagram of a nerve cell.
