Challenge
Moorings Park sought an online tool that
allowed patients and providers to
collaboratively track and manage
residents’ healthcare experience in order
to improve overall outcomes and quality
of care. They required a patient portal
application that could reside on a central
computer within the residents' homes.
The portal would allow residents to view
appointments and health reminders,
view medical records and provide
secured messaging channels between
patients and their healthcare providers in
order to improve care coordination and
collaboration. Additionally, Moorings
Park desired an application that
facilitated consumer engagement by
providing patients a wealth of easy to
use care management tools.
Solution
Softrim Corporation, a premier provider
of data, networking, computer consulting
and telephony services in Southwest
Florida, collaborated with Get Real
Health to develop a patient portal
solution. This solution used InstantPHRTM
along with the Microsoft® HealthVault®
platform serving as the primary data
store. Patients have access to a full
complement of patient engagement and
care management widgets within the
Moorings Park-branded portal. Because
the population consisted of senior
citizens who may have limited computer
literacy, ease of use was a high priority.
The provider portal allows caregivers to
monitor a resident’s health outside
facility walls while the administrator
portal can be used to set custom target
ranges and display educational materials,
based on a patient’s conditions,
medications, age, and lab results. This
solution uses an integrated single sign-
on solution between InstantPHRTM
and
the Aprima electronic health record
(EHR) system which enables patient-
generated data to be collected and fed
back into the EHR system.
Benefits
Not only do the residents of Moorings
Park benefit from InstantPHRTM
but
providers and staff of the community
as well. Providers now have the ability
to monitor and communicate with
patients without an office visit. By setting
alert rules and care plans for patients,
physicians can provide care for a wider
range of patients. Should residents
authorize it, the staff now have one place
to find vital health data in case of an
emergency. All of these practical
applications of InstantPHRTM
lead to
better health outcomes overall.
Relevance
As the first instance of InstantPHRTM
in a
retirement community setting, it is further
proof that everyone can benefit from
this user-friendly platform regardless
of computer literacy and experience.

Moorings Park Final Case Study

  • 1. Challenge Moorings Park sought an online tool that allowed patients and providers to collaboratively track and manage residents’ healthcare experience in order to improve overall outcomes and quality of care. They required a patient portal application that could reside on a central computer within the residents' homes. The portal would allow residents to view appointments and health reminders, view medical records and provide secured messaging channels between patients and their healthcare providers in order to improve care coordination and collaboration. Additionally, Moorings Park desired an application that facilitated consumer engagement by providing patients a wealth of easy to use care management tools. Solution Softrim Corporation, a premier provider of data, networking, computer consulting and telephony services in Southwest Florida, collaborated with Get Real Health to develop a patient portal solution. This solution used InstantPHRTM along with the Microsoft® HealthVault® platform serving as the primary data store. Patients have access to a full complement of patient engagement and care management widgets within the Moorings Park-branded portal. Because the population consisted of senior citizens who may have limited computer literacy, ease of use was a high priority. The provider portal allows caregivers to monitor a resident’s health outside facility walls while the administrator portal can be used to set custom target ranges and display educational materials, based on a patient’s conditions, medications, age, and lab results. This solution uses an integrated single sign- on solution between InstantPHRTM and the Aprima electronic health record (EHR) system which enables patient- generated data to be collected and fed back into the EHR system. Benefits Not only do the residents of Moorings Park benefit from InstantPHRTM but providers and staff of the community as well. Providers now have the ability to monitor and communicate with patients without an office visit. By setting alert rules and care plans for patients, physicians can provide care for a wider range of patients. Should residents authorize it, the staff now have one place to find vital health data in case of an emergency. All of these practical applications of InstantPHRTM lead to better health outcomes overall. Relevance As the first instance of InstantPHRTM in a retirement community setting, it is further proof that everyone can benefit from this user-friendly platform regardless of computer literacy and experience.
