The Enhancing Primary Care Programme aims to improve access to primary care services across Sheffield by expanding availability of services in the evenings, weekends, and through satellite units. The programme has established 4 satellite units that have provided over 30,000 additional appointments in their first year. These satellite units have improved access in more deprived areas and reduced emergency department utilization by an estimated 10-15%. The programme is working to better integrate services and develop primary care at larger scale to improve patient care and support primary care workforce challenges.
The world of grading shelley widdowosn final01PHEScreening
- In an ideal world, diabetic retinopathy grading would be 100% sensitive and specific, correctly identifying all true positives and negatives. However, in reality, grading has limitations and is not perfect.
- Screening programs experience false negatives and positives due to factors like lesions outside the image field, opacities masking lesions, subjective interpretation, and grader error or distraction. This means some patients are inappropriately referred or not referred.
- Variation in national referral rates may be due to inconsistencies in grading between screening programs. Improving grading quality through regular audits could help increase screening uptake by limiting false referrals and allowing extension of rescreening intervals for low risk patients.
This document summarizes an e-prescribing demonstration project conducted in rural Ontario. It provides background on e-health strategies in Ontario. The project was conducted at two rural health sites over 3 months, testing electronic prescribing between physicians and pharmacists. Kent has implemented telecare and telehealth programs since 2004. Over 1,100 users participated in telecare, which helped 93% feel more independent and safer at home. A telehealth pilot from 2005-2007 provided monitors to 250 older adults and found it reduced hospital visits, saved an average of £1,878 per patient over six months, and could potentially save £7,560,000-£10,942,000 annually for long term condition management in Kent. Mainstreaming assistive technologies faces challenges of making it a core service, equitable access, and changing commissioning practices to support assistive technology. The vision is for a shared 'platform' to facilitate wider use of technologies across users.
The document discusses the potential for a telemedicine pilot program to serve patients with rare diseases. It summarizes surveys that found over 50% of patients would be receptive to telemedicine and over 90% are willing to learn video call applications. The document estimates a 3-year pilot program could save £6.8 million in healthcare costs while costing approximately £975,000 to implement. Key operational requirements for the pilot include videoconferencing capabilities and secure data protection.
The document summarizes information about DrVisit.com, a telemedicine company founded in 2014. Its goals are to develop a telemedicine platform and provider network, create physician marketing and call center services, and promote widespread adoption of telemedicine to help address issues in the US healthcare system like rising costs and physician shortages. Overall life span (LS) was 1671.7±1721.6 days and cumulative 5YS reached 62.4%, 10 years – 50.4%, 20 years – 44.6%. 94 LCP lived more than 5 years without cancer (LS=2958.6±1723.6 days), 22 – more than 10 years (LS=5571±1841.8 days). 67 LCP died because of LC (LS=471.9±344 days). AT significantly improved 5YS (68% vs. 53.7%) (P=0.028 by log-rank test). Cox modeling displayed that 5YS of LCP significantly depended on: N0-N12, T3-4, blood cell circuit, cell ratio factors (ratio between cancer cells-CC and blood cells subpopulations), LC cell dynamics, recalcification time, heparin tolerance, prothrombin index, protein, AT, procedure type (P=0.000-0.031). Neural networks, genetic algorithm selection and bootstrap simulation revealed relationships between 5YS and N0-12 (rank=1), thrombocytes/CC (rank=2), segmented neutrophils/CC (3), eosinophils/CC (4), erythrocytes/CC (5), healthy cells/CC (6), lymphocytes/CC (7), stick neutrophils/CC (8), leucocytes/CC (9), monocytes/CC (10). Correct prediction of 5YS was 100% by neural networks computing (error=0.000; area under ROC curve=1.0).
