NOTES FOR FINAL PAPER
ASSESSMENT SUMMARY:
You will conduct an assessment on a character from a movie (there are several movies with addictions as the
primary topic). You will use the Biopsychosocial Interview and DSM Diagnosis (along with a brief statement
justifying your diagnosis). You will turn in a 2-3 page typed Assessment Summary and reactionto this assignment.
This assignment will be worth 50 points. This paper to be written in APA format. Follow these guidelines when
writing your summary and response. Attach this rubric below to your submitted assignment.
Assessment Summary
1. Describe your “client” in terms of their Biopsychosocial assessment.
2. Briefly note where your client falls on each of the six dimensions (ASAM)
3. Provide a DSM diagnosis for your client (Use the appropriate code from the DSM V)
4. Give a brief statement justifying yourDSM diagnosis.
Process Response
 Describe your reaction to completing the assessment.Possible topics to address:
o What aspects would be easy/hard about conducting the assessments?
 For you as the facilitator or for the client?
o What would be different if doing this assessment on a “real client?”
o How are assessments beneficial to the treatment process?
o How might assessments hurt/interfere with the treatment process?
o What would you add to or change about the assessment process?
TREATMENT PLAN:
Using the data you collected in the Assessment assignment you will diagnose yourclient and create an
individualized treatment plan. This section should be 2-3 pages long and this assignment will be worth 50 points.
 Note your areas of concern (problem list) for your client based on the results of their assessments.
 For each goal also include: indicators, short and long term goals, objectives and methods/interventions.
 Recommend an appropriate level of care for yourclient, and provide justification
*Style and Mechanics
 Appropriate Page Length (4-6 pages)
 APA formatting (look at online resources if you do not know what this means)
 Spell checked, no typos
 Appropriate structure:paragraphs (introduction, supporting paragraphs,conclusion),complete
sentences.See note below on further deductions.
 Style and flow of paper- can the reader follow the train of thought? Is there a logical transition between
paragraphs? See note below on further deductions.
*Paper Deductions- these items are not worth a point value, but carry a deduction if not complete
 Unstapled paper (two point deduction)
 Correct page length (deduction of five points per page that is short of requirement)
 Extreme grammatical/mechanical errors that impede reading the paper (more than a few mistakes,
makes it almost impossible to read and comprehend the meaning of the paper).
 Paper does not consist of structured paragraphs.
