Knee arthroscopy is a surgical procedure that uses an arthroscope to examine the inside of the knee joint. The procedure is performed with the knee bent and an arthroscope is inserted into the knee to allow the surgeon to see the structures inside the joint like the meniscus and ligaments. Common procedures include repairs of torn cartilage or ligaments.

Three cases of anterior cruciate ligament (ACL) injuries that occurred during indoor ball games are described. Case 1 involved an external rotation of the femur under varus load of the knee. Overuse shoulder injuries are common in overhead athletes such as swimmers, throwers, and racquet sport players. Epidemiological studies show that up to 50% of elite handball players and 67% of elite swimmers experience shoulder pain. Overuse injuries result from repetitive microtrauma to tendons, muscles, ligaments and bones from the overhead motion of these sports. Prevention focuses on reducing training errors and volumes, strengthening the scapular muscles and rotator cuff, and addressing biomechanical faults. Rehabilitation incorporates kinetic chain exercises, eccentric strengthening, and a gradual return to overhead activity.
Case Review #5: 62 year old male with degenerative disc disease C2-C6
62 year old male presented to Dr. Pashman after a previous fusion at C4/5. The patient had severe degeneration from C2/3-C5/6. Dr. Pashman treated the patient with an Anterior Cervical fusion followed by a Posterior Cervical Fusion.

There are several common injuries that can occur in javelin throwers, including injuries to the elbow, shoulder, and back. Key factors that can help prevent these injuries are having proper throwing technique, sufficient strength and conditioning of the whole body, adequate flexibility and joint range of motion, and supervision of athletes. Common elbow injuries include "thrower's elbow" which can result from improper technique producing excess strain on the ligaments. Shoulder injuries often involve displacement of the humeral head and can be addressed through strengthening exercises. Back injuries typically involve muscle strains or stress fractures from the alternating flexion and extension demands on the lumbar spine during the throw.

This study describes the safety areas of 5 lateral and 3 medial hip arthroscopy portals in relation to nearby neurovascular structures based on cadaver dissections. For the lateral portals, the anterior portal had the highest risk of injuring the femoral cutaneous nerve located 1.42 cm away on average. The medial portal with the narrowest safety margin was the anterior medial portal, located an average of 2.14 cm from the femoral artery laterally and 0.87 cm from the obturator nerve inferiorly, posing risks of injuring these structures. While the lateral portals generally had wider safety margins, the anterior portal specifically had the closest relationship to a neurovascular structure and therefore the highest risk of injury.

Anterolateral ligament in pediatric knees a radiographic study
Background:
The anterolateral ligament (ALL) is a true well-defined ligament in the knee first described in 1879 by Segond. After the work of Claes et al., several studies were conducted about biomechanics and its role in stability of the knee. The anatomical existence of the ALL has been studied by and various radiographic diagnostic modalities and in cadavers. It originates from lateral femoral epicondyle and is inserted between Gerdy’s tubercle and the fibular head. There has been controversy about the existence of ALL in pediatric patients. The aim of this work was to confirm the presence of ALL in pediatric patients by using MRI.
Materials and Methods:
We reviewed the knee MRI scans of 100 pediatric patients (ages between one and 12 yr) who had no knee injury or congenital deformity and had been evaluated by an expert radiologist.
Results:
The ALL was detected in 90% of the pediatric patients with the use of MRI.
Conclusions:
The main finding of this study was that ALL can be seen in pediatric patients using MRI. Despite numerous studies, additional research is needed to further define the role of the ALL in knee function.

Case Review #3: 53 year old male with severe spinal cord compression
A 53-year-old male presented with severe cervical kyphosis centered at C4-5, C5-6, and C6-7, causing spinal cord compression and myeloradiculopathy. Pre-op x-rays showed 78 degrees of kyphosis. The surgical strategy involved segmental spinal instrumentation from C4 to C7, radical diskectomies and deformity correction through osteotomies and vertebrectomies at C4-5, C5-6, and C6-7. Post-op x-rays showed successful reconstruction of natural cervical alignment and removal of 78 degrees of kyphosis.

The MLG-R system classifies injuries based on their mechanism (M), location (L), relation to tendons/connective tissue (G), and whether it is a recurrent injury (R). Previous classification systems are reviewed which focused on grading or specific muscles. The new system aims to be reproducible, distinguish different injury categories, be easy to remember, and correlate to prognosis. Magnetic resonance imaging and ultrasound are important for accurately describing the location, size and tendon involvement of injuries. The MLG-R system is designed for hamstring injuries but could be expanded to other muscles.
A Study on 3D Finite Element Analysis of Anterior Cruciate Ligament Behavior ...
The present study deals with the force and stress distribution within the anteromedial (AM) and posterolateral (PL) bundles of the anterior cruciate ligament (ACL) in response to an anterior tibial load with the knee at full extension was calculated using a validated three dimensional finite element model (FEM) of a human ACL. The interaction between the AM and PL bundles, as well as the contact and friction caused by the ACL wrapping around the bone during knee motion, were included in the model. The AM and PL bundles of the ACL were simulated as incompressible homogeneous and isotropic hyperelastic materials. The validated FEM was then used to calculate the force and stress distribution within the ACL under an anterior tibial load at full extension. The AM and PL bundles shared the force, and the stress distribution was non-uniform within both bundles with the highest stress localized near the femoral insertion site. The contact and friction caused by the ACL wrapping around the bone during knee motion played the role of transferring the force from the ACL to the bone, and had a direct effect on the force and stress distribution of the ACL. This validated model will enable the analysis of force and stress distribution in the ACL in response to more complex loading conditions and has the potential to help design improved surgical procedures following ACL injuries.

Hip dislocation and_femoral_neck_fracture_decision
This document describes a case report of a 31-year-old man who suffered a hip dislocation and femoral neck fracture in a motor vehicle accident. During surgery, the integrity of the medial femoral circumflex artery and retinacular vessels was assessed to determine whether osteosynthesis or joint replacement should be performed. Observation of the intact artery and vessels allowed for fixation of the fracture with screws. Follow-up angiography and bone scans confirmed adequate blood supply to the femoral head. However, signs of implant failure later emerged, requiring revision surgery. The case report demonstrates the importance of intraoperative assessment of vascular structures in deciding between head-preserving or replacing procedures for this injury pattern.

Tensiomyography (TMG) is a diagnostic tool that evaluates muscle contraction properties. It can be used for injury prevention and rehabilitation monitoring in athletes. TMG provides immediate feedback on how individual muscles contract through a non-invasive test. This allows for identification of muscle imbalances that could increase injury risk. TMG data on contraction symmetry is valuable for determining if an injured athlete is ready to return to play by comparing the injured muscle to the uninjured side. TMG can also monitor muscle adaptation during rehabilitation, such as providing objective data on muscle recovery progress after ACL surgery.

This study aimed to investigate the effects of elastic adhesive bandage (EAB) and zinc-oxide (ZO) tape on ankle proprioception in amateur footballers. Sixteen male university football players participated in a protocol involving pre-exercise questionnaires, taping with EAB or ZO, a warm-up, and a 30-minute agility exercise. Proprioception was measured pre- and post-exercise using a Biodex Balance machine. While results are still being analyzed, it is expected that taping will positively influence ankle proprioception and that EAB or ZO tape may differ in the level of support provided. This study aimed to investigate the effects of elastic adhesive bandage (EAB) and zinc-oxide (ZO) tape on ankle proprioception in amateur footballers. Sixteen male university football players participated in a protocol involving pre-exercise questionnaires, taping with EAB or ZO, a warm-up, and a 30-minute agility exercise. Proprioception was measured pre- and post-exercise using a Biodex Balance machine. While results are still being analyzed, it is expected that taping will positively influence ankle proprioception and that EAB or ZO tape may differ in the level of support provided. Cell Therapy Expansion and Challenges in Autoimmune Disease
There is increasing confidence that cell therapies will soon play a role in the treatment of autoimmune disorders, but the extent of this impact remains to be seen. Early readouts on autologous CAR-Ts in lupus are encouraging, but manufacturing and cost limitations are likely to restrict access to highly refractory patients. Allogeneic CAR-Ts have the potential to broaden access to earlier lines of treatment due to their inherent cost benefits, however they will need to demonstrate comparable or improved efficacy to established modalities.
In addition to infrastructure and capacity constraints, CAR-Ts face a very different risk-benefit dynamic in autoimmune compared to oncology, highlighting the need for tolerable therapies with low adverse event risk. CAR-NK and Treg-based therapies are also being developed in certain autoimmune disorders and may demonstrate favorable safety profiles. Several novel non-cell therapies such as bispecific antibodies, nanobodies, and RNAi drugs, may also offer future alternative competitive solutions with variable value propositions.
Widespread adoption of cell therapies will not only require strong efficacy and safety data, but also adapted pricing and access strategies. At oncology-based price points, CAR-Ts are unlikely to achieve broad market access in autoimmune disorders, with eligible patient populations that are potentially orders of magnitude greater than the number of currently addressable cancer patients. Developers have made strides towards reducing cell therapy COGS while improving manufacturing efficiency, but payors will inevitably restrict access until more sustainable pricing is achieved.
Despite these headwinds, industry leaders and investors remain confident that cell therapies are poised to address significant unmet need in patients suffering from autoimmune disorders. However, the extent of this impact on the treatment landscape remains to be seen, as the industry rapidly approaches an inflection point.
