At the end of the 2nd day of this conference on serious gaming for health I’ll address one last serious issue. KLIK
- More often than not innovations in health care lead to low value care. Only if they contribute to appropriate care they will realize high value care and eventually find structural financing and reimbursement. But what exactly is appropriate care when it comes to development and implementation of games in health? A view from the Dutch National Health Care Institute . KLIK.
- Affiliated with ZIN [POLICY] Every person in the Netherlands is entitled to health care offered in the basic health insurance package. We all contribute to this through our health insurance. The National Health Care Institute determines and advises on which types of health care are included in the basic care package and which are not. Taking care of good healthcare – no more and no less than necessary. The philosophy underpinning the Dutch healthcare system is based on several more or less universal principles: access to care for all, solidarity through medical insurance (which is compulsory for all and available to all) and high-quality healthcare services. The Dutch system has been shaped by a number of historical trends, developments and social conditions. The National Health Care Institute (Zorginstituut Nederland) carries out tasks relating to two Dutch statutory health insurance schemes: the Health Insurance Act (Zorgverzekeringswet) and the Long-Term Care Act (Wet langdurige Zorg, Wlz). The National Health Care Institute’s role in maintaining the quality, accessibility and affordability of health care in the Netherlands involves 4 tasks.

De zorguitgaven verdubbelen tot 174 miljard euro in 2040. In de periode 2015-2040 groeien de zorguitgaven met gemiddeld 2,9 procent per jaar. De zorguitgaven per persoon stijgen van 5.100 euro in 2015 naar 9.600 euro in 2040.
- In het Trendscenario stijgen de reële zorguitgaven tot 174 miljard euro in 2040. Dat is een verdubbeling ten opzichte van 2015. Met de uitdrukking ‘reële zorguitgaven’ wordt bedoeld dat in de projectie van de uitgaven de ontwikkeling van prijzen niet wordt meegenomen. -directe medische zorguitgaven en omvat daarom zowel collectieve als private uitgaven. -Uitgaven aan kinderopvang, welzijn en jeugdzorg zijn niet meegenomen. De toekomstverkenning is gebaseerd op een analyse van de zorguitgaven in de periode 2003-2015. In deze periode zijn de reële zorguitgaven toegenomen met gemiddeld 2,8 procent per jaar. In de recente periode 2013-2015 is er sprake geweest van een sterke kostenbeheersing. 2015-2040: 2,9% per jaar
- In het Trendscenario stijgen de zorguitgaven per persoon van 5.100 euro in 2015 tot 9.600 euro in 2040. Deze uitgaven bestaan uit een deel dat collectief gefinancierd wordt en een deel dat op andere manieren gefinancierd wordt; de overige uitgaven. Dit zijn voor een belangrijk deel eigen bijdragen en aanvullende zorgverzekeringen. De collectieve uitgaven stijgen van 3.800 euro per persoon in 2015 naar 7.700 in 2040, de overige uitgaven van 1.200 euro in 2015 naar 1.900 in 2040. Hoeveel zorg er wordt betaald uit collectieve middelen en hoeveel uit eigen betalingen of aanvullende zorgverzekeringen is een beleidsmatige keuze. Het Trendscenario gaat uit van de veronderstelling dat er geen nieuw of geïntensiveerd beleid wordt ingezet. De toekomstige verhouding tussen collectieve en overige betalingen is daarom hetzelfde als de verhouding in de laatste beleidsbegroting.
- Van de groei van de totale zorguitgaven (2,9 procent) is in het Trendscenario een derde deel (1,2 procent) gebaseerd op demografische ontwikkelingen. Twee derde van de uitgavengroei (1,7 procent) is toe te schrijven aan andere ontwikkelingen, zoals een veranderend zorggebruik als gevolg van medische technologie en welvaartstijging.

Met green deals wil de overheid nu de transitie naar een duurzame samenleving inzetten: uitstoot verminderen, energie verduurzamen in/met alle sectoren, dus ook in de gezondheidszorg. - Innovation in health care is hard Relatie tussen tech en stijging zorguitgaven is complex. Dubbel: 1. Tech leidt tot meer Zorguitgaven: dure medicijnen/kanker, operatie-robots, e.a. med tech; hogere vraag, maar op termijn beetje duurder want meer patienten/intensiever; vaak tijdelijk (als uitvoeringskosten, scholingskosten gedaan zijn, patenten zijn verlopen, concurrentie); implementatie/innovatie vraagt veel van organisatie 2. Tech leidt tot minder zorguitgaven: kosten dalen door kwaliteitsverbetering (net als telefonie), minder personeel Remember: the biggest part of healthcare is still about human dialogue: information, consolation, encouragement, support. Centrale vraag van het onderzoek is: wat is het meest effectieve overheidsbeleid om de totstandkoming van doelmatige innovaties in de zorg en de implementatie daarvan te bevorderen?
