This document provides a template for a performance appraisal form. The form is divided into two parts: Part A is completed by the employee and covers things like their understanding of duties, achievements, likes/dislikes about their job, goals for the next year, and self-assessment of skills. Part B is completed during the appraisal meeting and includes discussing the employee's job purpose, reviewing objectives and skills assessments, agreeing on development goals and objectives for the next period. The template provides guidance for appraisers to have a structured discussion that identifies training needs and sets goals.
