“metabolic syndrome” with the advantage of remote monitoring the patient progress with

Solution Overview
 High Level Solution Description:
 The goal of this App is for the doctor and/or the individual develop a wellness/preventive
program that tackles the “metabolic syndrome” (Hypertension, Reversible diabetes, High
Cholesterol Levels and Obesity) using 8 simple steps, with information provided by the doctor
and/or the individual as well as the government through a link to Healthfinder.gov

 This App allows doctor/individual to track remotely progress done during the program,
targeting the following final goals:
• Reduction in prescription drugs (in some cases no more medication)
• Close to normal, if not normal, biometric parameters used to monitor progress for the individual. Some
examples are:
• Weight, BMI, Blood Pressure, Glucose Levels and others parameters
• A High level of EQ (Emotional Quotient) for better self-worth

 The App will be developed both in English and Spanish thus targeting the Spanish-speaking
market in the Americas

Solution Overview
 Healthcare/Business problem addressed:
 This Apps allows for the close interaction between the doctor and individual (tracking progress remotely
anywhere anytime) that suffers from various aspects of the “metabolic syndrome” (Hypertension,
Obesity, Diabetes, High Cholesterol and others) with the final outcome of better care, better health as
patients improve their health and change their behaviors, and reduction of costs in drug treatments, re-
admissions and overall support. It also address all the ages from seniors, adults to children. This is the
ultimate goal of “managed care” with positive outcomes.

 The application developed addresses the following category:
 Applications that improve management of high cost chronic diseases.
 Innovative approaches to addressing value-based care imperatives.
 Both: This application will serve both categories.


Usability and Design
 How this App will be used:
• Healthcare provider, with patient inputs, develops a program that monitors both the medical prescription
usage and the wellness activities. As the wellness activities monitoring system measures over 90%
accomplished, medication can be reduced or stopped completely if all the vital parameters are
normalized or read very close to normal level
• During the program, the patient will check in with the Healthcare provider at certain intervals of times for
consultation but eventually the intervals will be done away since the patient’s vitals would have returned
to normal levels as much as possible
• The patient EQ (self-worth) will be quite high since a new healthier lifestyle of living is achieved and there
is more energy for daily activities
• Re-admission costs are reduced, prescription drug cost are reduced, productivity and moral go up thus
being a win-win situation for Healthcare provider and patient


Development Stage
 Progress done up to date:
 Idea : It has been developed and early content is being worked on
 Prototype : We have a small example of what it may look like as shown in the Video Presentation
 Code development in progress : Some Code has been written.

 Integration into the AllScripts SDK:
 Reading the AllScripts Integration documentation and writing overall software architecture/implementation
spec
 A large portion of the Apps content has been documented and parts of it has been implemented

 Still work in progress in understanding the full integration with AllScripts solution. We could not attend the
presentation given here in California since we knew about this challenge a few days before the Christmas
holidays but are ramping up quickly

 It is a very comprehensive Apps that will allow for customized wellness program,
(including preventive information provided by the government through a
Healthfinder.gov link) that is very interactive between the patient and doctor. In addition,
both the patient and doctor, can monitor progress remotely with the mindset of making
lifestyle changes for a healthier person and reducing the long term costs of the patient’s
care to the health care system
