Solution Overview
  High Level Solution Description:
         •   Problem and medication specific patient education, delivered via mobile app, and
             shareable with caregivers. Education is focused on the transition of care from hospital to
             the ambulatory setting, with the goal of preventing readmission. Education is easily
             customized to better align recommendations with actual practice, and incorporates text
             to speech so patients can hear the mobile application speak to them which has a greater
             likelihood of comprehension versus plain text for patient’s to read.
  Healthcare/Business problem addressed:
         •   1300 hospitals in the US received a penalty of up to 2% (over $900M)as part of the
             Medicare Value Based Purchasing initiative. Among the many metrics related to quality,
             is patient satisfaction, and hospital readmission rates. Often, health literacy is an
             obstacle to better patient understanding of discharge instructions that might prevent
             readmission, and this mobile app hopes to address this issue.
  The application developed addresses the following category:
          Applications that improve management of high cost chronic diseases.
         X Innovative approaches to addressing value-based care imperatives.

Usability and Design
 Problem specific patient education is prescribed along with all discharge
     orders by the physician. Upon placement of order, the EHR will send the
     patient’s problem and medication list to the mobile app

 Patient is provided with a “Prescription” for education containing URL,
     Username (their email), and Password

 In addition to problem and medication specific education, the mobile app
     will include transition of care tools, such a provider directory to receive
     follow up specialty care

Why this solution be selected!
 This solution not only addresses an important indicator of hospital care
     quality (readmission rates), most importantly it will help overcome
     health literacy challenges, and empower patients (as well as their care
     partners) with knowledge to better manage their own medical conditions.
     In fact, because it is easily viewed and shared with mobile devices, it is
     quite possible that this might facilitate participation of care management
     but an ederly patient’s younger family members (children and
     grandchildren) providing them with knowledge to care for their loved
     ones, as well as insight to help prevent disease in themselves and
     immediate family.
