The album cover references a car accident the band had while driving to film a music video. It depicts a van in a snowy field. The title is taken from a letter Vincent van Gogh wrote to his brother describing his renewed health and vigor for painting. "Folie à Deux" is a French term meaning a madness shared by two, referring to how the bear thinks he's a man and the man thinks he's a bear. It was the first album by the band to feature photos of themselves on the cover, dressed as cowboys and taken during a photo session in a sandpit. All the band's previous albums featured the War-Pig symbol, a f
