I, DEEMS EVAN T. MENGULLO, of legal age, Filipino, with
residence and postal address at Mapanas, Northern Samar, after
having been duly sworn to in accordance with law depose and say:
That I am presently employed at Mapanas Agro Industrial
High School, as Teacher I;
That I am designated as Assistant Coach for athletics
(Secondary Boys) in the upcoming Sports Competition ( Eastern
Visayas Regional Athletic Association) to be held on January 26-
February 2, 2013, at Tacloban City;
That I have submitted credentials or documents pertaining to
the qualifications of my athletes to the Department of Education;
That said credentials or documents are valid, authentic and
correct;
That I am executing this affidavit to attest to the truth of the
foregoing circumstances and to attest to the validity and
authenticity of the documents submitted.
