This document contains a questionnaire for conducting a household survey. The questionnaire contains 35 questions organized into several sections on household information, family members, education, occupation, land ownership, livestock, sources of income, marriage customs, housing, social and cultural practices, and the impacts of modernization. The questions are mostly multiple choice to collect demographic and socioeconomic data about households.
