The rash could be consistent with a diagnosis of Rocky Mountain spotted fever, with central nervous sys tem involvement, but patients with tickborne rick ettsial diseases frequently have cytopenias and elevated liverfunction tests.
４４ ロッキー山紅斑熱 （１） 定義 紅斑熱群リケッチアに属するロッキー山紅斑熱リケッチア(Rickettsia rickettsii)による感染症である。 （２） 臨床的特徴 自然界ではダニ、げっ歯類、大動物（イヌなど）の間で感染環が維持されている。ヒトへの感染はダニの刺咬による。潜伏期間は3～12日であり、頭痛、全身倦怠感、高熱などで発症する。通常、つつが虫病などでみられるような刺し口は生じない。高熱とほぼ同時に、紅色の斑丘疹が手足などの末梢部から求心性に多発し、部位によっては点状出血を伴う。ときにリンパ節腫脹がみられる。その後、中枢神経系症状、不整脈、乏尿、ショックなどの合併症を呈する。診断・治療の遅れ、高齢者、発疹がみられない、ダニの刺咬歴がある、冬季の発症などでは、致死率が高い。

As the discussant mentioned, evaluation of cerebrospinal fluid typically shows a lymphocyt ic pleocytosis; however, a neutrophilic predomi nance occurs in as many as 40% of cases.
Rash has been described in approximately 20% of pa tients infected with West Nile virus.3,4
IgM antibodies are detectable in the serum of 98% of patients after the first week of illness6 but may
persist for more than 500 days after acute infec tion.7
The hallmark of Rocky Moun tain spotted fever is a petechial skin eruption, beginning as blanching erythematous macules around the wrists and ankles and progressing to involve the trunk and sometimes the palms and soles (Fig. 2).
A rise in the IgG antibody titer by a factor of at least 4 between serum specimens obtained 2 to 4 weeks apart is required for laboratory confirma tion of acute infection.
A skin biopsy may provide a more timely diagnosis; immunohistochemical staining for R. rickettsii is 70% sensitive.11
