DiabetesAlert Day is on March 22nd and encourages everyone to take a one minute test to check their risk for Type 2 diabetes. The Diabetes Referral Network in Central Nebraska has developed a coordinated system of care for those with pre-diabetes and diabetes by creating a Diabetes Education Referral Pathway to increase referrals to local resources. The coalition holds an annual Diabetes Information and Referral Fair to showcase local resources and screenings for community members and providers.

Over 86 million Americans have prediabetes, but less than 10% know they are at risk. This new initiative will expand screening and refer more people to diabetes prevention programs. It provides tools for doctors to screen and refer patients to local programs. Diabetes and the health care safety net
Diabetes is a growing global health issue affecting 346 million people worldwide. In the US, over 550,000 people with diabetes rely on community health clinics as their primary source of care due to cost barriers. As the recession has increased the number of uninsured, community clinics are seeing a rise in diabetic patients but have limited resources to support growing demand. A recent survey found a 7% increase in uninsured diabetic patients at community clinics from 2010 to 2011, totaling an additional 36,000 new patients. These safety net providers are critical in ensuring consistent and affordable care for millions of Americans with diabetes.
Bridging Clinical Gaps and Disparities in Care in TNBC
This webinar will focuses on racial, ethnic, and socioeconomic disparities with the clinical gaps in treatment for women with triple-negative breast cancer (TNBC). Our guest speaker Shonta Chambers, MSW, is the EVP of Health Equity and Community Engagement at the Patient Advocate Foundation and Principal Investigator for SelfMade Health Network. Come and learn about this complex subtype, barriers to care, address the myths and fears around clinical trials in specific racial and ethnic communities, and help bridge the clinical gaps to improve survival outcomes for patients with TNBC.

Bridging the Gap White Paper
Over 12 million women in the US have diabetes. A study found that while type 1 and type 2 diabetes have clinical differences, women with both types face similar challenges in managing their condition on an emotional and psychosocial level. They share motivators for managing their health, challenges in daily management like lifestyle behaviors, and approach problem solving similarly. The findings suggest healthcare programs should focus on these shared psychosocial needs rather than separating support based on diabetes type alone.
Culturally Specific Programs Urged in Diabetes Care
Diabetes remains a serious problem in the US, especially for certain ethnic and cultural groups who may not be receiving appropriate treatment. Physicians must work with community leaders to develop culturally-sensitive education programs about diabetes risks, healthy lifestyle changes, and traditional food preparation. Experts stress that education is key to reducing diabetes rates in at-risk communities by pairing lessons with cultural activities in places like religious centers. While diabetes cases are projected to rise significantly, medical professionals are making progress in combating the disease through prevention and management efforts.
American diabetes association
The American Diabetes Association is a non-profit organization founded in 1940 to fund research and provide services related to diabetes. Their mission is to prevent and cure diabetes and improve the lives of those with the disease. The executive director has a personal connection as diabetes has affected his family. The organization relies on donations to run programs that promote awareness, fund research, and help manage diabetes. Their goal is to find better ways to prevent, detect, and treat diabetes to help people live longer, happier lives.

KPGA-MDSP-YMCA story
The Kaiser Permanente Georgia region partnered with the YMCA and Medicare Diabetes Screening Project to help recruit participants for a diabetes prevention program. KPGA was able to identify over 250,000 members who were pre-diabetic using its electronic medical records. It recruited nearly 120 additional participants for the program by sending letters to eligible members. One participant was able to lower her blood sugar levels and lose 18 pounds through the program. The results of the pilot program will be presented to Congressional policymakers to discuss expanding Medicare coverage of similar community programs.
