sub-contractors;
• 2015-03 review & update HSE training material;
• 2015-06 plan to focus on office employee training...

• 2 of 4 head injury related with “stood up”,
another 2 are improper operation;
• 5 Hand injury related with “small
transportation”, “Tools”, “Slip”;
• 2 of 12 injury happened in 2nd shift.
Among 7 injury in Q3
• 1 head injury
• 3 hand injury (because of platform door &
cabinet door ) & one highlight (by chain)
• 1 heatstroke (non reportable accident)

Total Near Miss Analysis
Among 20 near miss reporting Q1~Q3
• 9 near miss because of internal transportation, 4 forklift & 5
crane. (meeting with cranes operators & riggers/done)
• 3 waste burning because of operators pay less attention to
the environment.
• 1 significant near miss in 13th July. (Press Machine,
corrective action done)

  • 18. 18 Total Near Miss Analysis Among 20 near miss reporting Q1~Q3 • 9 near miss because of internal transportation, 4 forklift & 5 crane. (meeting with cranes operators & riggers/done) • 3 waste burning because of operators pay less attention to the environment. • 1 significant near miss in 13th July. (Press Machine, corrective action done)
