5. IT Enabler Board - the commitment to make it
happen
• IT Enabler programme board with representation from all care
providers and Healthwatch
• Agreed set of core principles to support the IT enabler investment
• Delegated authority to IT enabler board and project teams within
agreed digital model
• Joint workshops across health and social care professionals to
understand the information exchange requirements
• Joint provider/supplier discussions across all providers and their
respective system supplier
• Close working with NHS Digital to secure social care integration
(support the implementation of NHS numbers and spine services)

• Population Health – advanced analytics to understand the current
demand and model future services; to help prevent poor health and
deterioration
• Extending the interoperability agenda:
o enhancing the connections across Hackney
o connecting information exchanges across London to support the whole
patient journey
o digital image sharing to support the health system in Hackney and
extending to the London cancer network
o social prescribing and improving links with the voluntary sector
o “Digital Discharge to Social Care”: real-time electronic communication of
assessment and discharge notifications to social care
