Mr. and Mrs. Faurah, an immigrant couple in their early 70s, have been living with their oldest
daughter and her husband of 15 years and their three children. They live in a middle-income
neighborhood in a suburb of a metropolitan city. Mr. Faurah attended college and Mrs. Faurah is
high school educated. Mr. Faurah worked fulltime, and Mrs. Faurah stayed at home raising their
children (total of 3) in their native country. They came to the United States after their oldest
daughter became a U.S. citizen following her marriage to her husband, who she met in college.
The Faurahs found life in the United States very different from their native country in Saudi
Arabia and continue to adjust. They are both overall healthy, Mr. Faurah speaks fluent English
and Mrs. Faurah continues to work on the language. Most of their time has been spent taking
care of the grandchildren in the house but the grandchildren have now grown older and their
daughter and her husband have advanced their careers and spend much of their time working.
Mrs. Faurah has been fighting a respiratory illness for around 4 weeks. She presents to the
Urgent Care.
Blood pressure 130/88, RR 20, HR 120, O2 saturation 94%, weight 155, height 53
Diagnosis from physician: Pneumonia- she is given a prescription, steroids, and is being sent to
her Primary Care Provider for follow-up in three days.
You are the RN caring for Mrs. Faurah at the follow-up appointment. Her Blood Pressure is now
132/86, HR 115, HR 100, and O2 saturation is 96%. She states she is feeling much better and the
provider has determined her condition has improved. She is encouraged to follow-up again in
two weeks to ensure the pneumonia has cleared up and provided information on where to go/who
to call is her condition worsens.
Upon additional conversation, Mrs. Faurah states that that she is feeling the loss of her
role/confusion about her identity as a mother and as a grandmother due to the independence of
her grandchildren and her daughter. Mrs. Faurah has been an occasional smoker her entire life;
however, within the last six months she has started to smoke regularly again and is up to pack
/day. You open her chart and note that she has gained 20 lbs. in the last year.
Family history shows:
Maternal grandmother (I.S) died at 85 from heart attack
Maternal grandfather (W.S.) died at 66- unknown reasons
Paternal grandmother (S.F) died at 55 from breast cancer, diagnosed at 54
Paternal grandfather (P.F.) died at 78, suffered from diabetes, heart disease, and died in his sleep
Her mother (R.F.) died at age 49 from heart disease and had high blood pressure
Her father (R.F.) died at age 79 from lung cancer and also had high blood pressure and high
cholesterol
She (M.F.) has one brother (F.F.) who is currently alive at age 77 who is obese and has diabetes
Her last visit to the primary care clinic before the Urgent Care visit was four years ago where she
had a blood pressure of 120/83, and she weighed 135, height was 53

blood pressure medication.
Mrs. Faurahs assessment continues. She states that her daughter and husband are not aware of
her increase in smoking habits and that it would make them very upset if they knew. She also
tells you that she once was walked regularly in their neighborhood, but due to the cold weather
and the increase in crime in the area she no longer feels comfortable or safe doing so. Mrs.
Faurah breaks down and begins to cry during this visit and asks what you can do to help her.
