- Disability in children has a wide impact on communities globally. This impact includes social, emotional, educational health and financial effects. There is an urgent need to identify and provide early intervention to children with a disability. We also have humanitarian responsibilities to all children, not only those with disabilities, but also the Children of Tomorrow.
- For the last 20 years, Hear and Say has been overturning existing doctrine regarding how deaf children can listen and speak, and have become a leader in the field of paediatric hearing loss.
- Up to 760,000 children born with hearing loss every year!
- My dream – my impossible dream – is to deliver listening and spoken language communication to all deaf children world-wide.
- We stand at that point in time where we have the potential to decimate the very significant impact of deafness worldwide through the wonders of technology!
- Newborn hearing screening and new hearing technology have caused the changes.
- Newborn hearing screening allows us to access babies’ brains early.
- Degree of deafness and type of deafness does not determine the outcomes for the child (except in some cases of auditory neuropathy and multiple disabilities). For the first time in history we can give parents the outcome they choose in almost every case!
- What has changed? Hearing loss is not about the ears; it’s about the brain! Intervention is not about “listening”, it’s about brain development. Hearing aids, FM systems and cochlear implants are not about the ears; they are about accessing the brain!
- We have only a small window of time to access the auditory brain to achieve optimal outcomes!
- ACCESS TO THE AUDITORY BRAIN AND INTERVENTION MUST START EARLY because every day wasted is a day lost in building the auditory brain.
- For babies with hearing loss, it is a different story: 20 weeks of auditory deprivation, no auditory brain development.
- This rapid development continues after birth but BUT ONLY IF THE auditory brain is stimulated. (Kral et.al., 2002; Vouloumanos & Werker, 2004)
- We need to develop their auditory brain pathways from a rutted track to 8 lane highways. Babies need abundant meaningful sound experiences to consolidate the neural wiring and build more connections (Cole & Flexer, 2007) Auditory cells are highly tuned to receiving and processing auditory stimulation
- Centre for Cognitive Neuroscience Structure and function is different to visual cells – visual cells decrease in branching density and membranes Auditory cells flourished and grew new until 7.5 months in macaque monkeys.
- Audiologic and education intervention
- What types of sounds attracts newborns to listen? Complex auditory stimuli, rich in special characteristics or temporal patterns (Werker, 2007).
- Why is it a Neurological emergency when a deaf baby is diagnosed? Much brain maturation has taken place before birth in normal hearing babies over 20 weeks. By birth they are already good listeners.
- Over the first year, these initial sensitivities become increasingly tuned to properties of native language. (Werker & Tees, 1992)
- At 17 months, m onolingual babies can differentiate phonetically similar words (Werker, et al., 2002; Mills, et al., 2004). Bilingual babies can do this by 20 months (Fennel, et al., 2007). Decline in aspects of speech not pivotal for processing native language (Kuhl, et al., 1992) Gain in sensitivities pertinent to native language processing (Jusczyk, et al., 1999) Reduced ability to discriminate between sounds of own language and another language (Werker & Tees, 1984)
- Difficulties in using the rich auditory information a baby hears can predict later language delay (Bernhardt et al., 2007; Molfese, 2000; Tsao et al., 2004).

- “ Parent Choice” of an education option at birth does not result in similar outcomes for all deaf children BECAUSE…….. DELAY in or LACK of stimulation of auditory brain results in diminished capacity for auditory brain growth. (Sharma, et al., 2005) DEPRIVATION OF AUDITORY INPUT RESULTS IN SERIOUS CENTRAL NERVOUS SYSTEM DEFICITS (Kral, et al., 2000; Klinke, et al., 2001).
