• His right index finger had to be surgically
amputated.

man died Friday afternoon after
falling out of the back of a pickup
truck and hitting the pavement in
Orange Park, according to the Florida
Highway Patrol.
• Troopers said two workers were in

Park when one fell out and hit the
pavement.

• Willie Wakefield, 56, sued after a pile of
lumber fell three stories from a forklift at a
Chicago construction site and struck him in
the head.
• The March 2005 accident left Wakefield with a
concussion and a diminished capacity for
memory and learning.

rig when they were being

weigh about 70 pounds each.

after an accident with a piece of
heavy paving machinery.

42-year-old man became trapped
in the paving machine around

High wind

employee from an elevated

employee or equipment handling
material to lose control of the
material, or
• The wind would expose an
employee to other hazards not
controlled by the standard
involved.

• The man was measuring the
grade of the road, authorities at

grader did not see the man, and
called the incident a “horrible
accident.”
Compactor

• The operator parked the compactor on the
road shoulder that had been created during
the compacting process which was
approximately 10 to 12 inches higher than
the compacted road.
• After returning from his lunch break, the
operator was in the process of backing up
the compactor to continue with his work.
The operator turned the compactor tires in
the direction of the road in order to get off
the shoulder when the left tire slid off the
shoulder onto the road causing the
compactor to roll over, pinning him between
the road and the top edge of the roll over
protection system/cab cover.

• Ana Flores, 37, died when a
jagged shard of glass broke free
from a cracked window on the
29th floor of the CNA Plaza's
South Tower, at 333 S. Wabash
Ave., and smashed into her head
as she walked along the sidewalk,
holding the hand of her 3-year-
old daughter.
• Consultants hired by CNA found
that the building's glass could not
withstand thermal stress, which
occurs when a warm area of glass
expands against a cooler area,
creating pressure that can cause
it to crack.
• At least one study, performed
after another pedestrian was hurt
by falling glass in 1994,
recommended the building's
windows be replaced or
reinforced with a restraint system
that would hold cracked glass in
place until it could be fixed.
• CNA did not follow those
recommendations.

  • 83. • The owner of a Loop skyscraper where a woman was killed by falling glass in 1999 has agreed to pay her family $18 million • Ana Flores, 37, died when a jagged shard of glass broke free from a cracked window on the 29th floor of the CNA Plaza's South Tower, at 333 S. Wabash Ave., and smashed into her head as she walked along the sidewalk, holding the hand of her 3-year- old daughter. • Consultants hired by CNA found that the building's glass could not withstand thermal stress, which occurs when a warm area of glass expands against a cooler area, creating pressure that can cause it to crack. • At least one study, performed after another pedestrian was hurt by falling glass in 1994, recommended the building's windows be replaced or reinforced with a restraint system that would hold cracked glass in place until it could be fixed. • CNA did not follow those recommendations.
