CARDIOVERSION
• Cardioversia inseamna administrarea unui soc
sincron pe unda R a complexului QRS
DEFIBRILLATION
• Defibrilarea este administrarea unui soc
asincron , in orice faza a ciclului cardiac.
PRINCIPLE
• Energia trece de la polul negative la polul
pozitiv, iar inima este situate intre cei 2
• Reseteaza toate celulele miocardice, intrerupe
orice circuit electric anormal sau focar
aberrant, incat dupa soc isi reia activitatea
normal nodul sinusal.
• Nodul sinusal redevine suveranul care
controleza intreaga activitate a inimii
Inima este situate intre cei 2 poli:
negativ si pozitiv
Prima defibrilare la om.
Filmulet cardioersie cord deschis
Patchuri de defibrilare, diferite fata de
padelele clasice.

METODA
• Cardioversia este aproape intotdeauna efetuta sub
analgosedare: agenti cu actiune scurta de tipul
propofolului, midazolamului si mialginului.
• Singura exceptie este cazul pacientilor cu instabilitate
hemodinamica sau iminenta colapsului
• Defibrilarea este o manopera de urgenta si se poate
efectua fara sedare, sau in timp de se administreaza
sedarea, pentru rapiditatea manioperei.
Se pot folosi atat padele de defibrilare,
cat si patchuri autoadezive.
Unde se plaseaza electrozii ?
PLASAREA PADELELOR
• Pozitie antero-laterala, se aseaza una in
spatial 4 sau 5 intercostal stang pe linia
medioaxilara iar cealala sub clavicular dreapta,
de partea dreapta a sternului in spatial 2 sau 3
intercostal drept.
• Pozitie antero-posterioara, una este plasata
in dreapta sternului ca si mai sus iar cealalta
intre coloana vertebrala si scapula stanga.

ANTEROLATERAL SAU
ANTEROPOSTERIOR ?
• Daca copilul are pacemaker ?
• In cazul in care copilul prezinta fibrilatie atriala,
pozitionarea antero-posterioara poate fi
superioara celei antero-laterale deoarece atriul
stang este prins chiar intre polul negative si cel
pozitiv in cazul pozitionarii AP.
• Pozitionarea antero-posterioara este de
asemenea preferata la copiii cu pacemaker sau
defibrillator implantat in pozitie pectoral, cu
scopul de a evita deteriorarea aparatului in cursul
defibrilarii/cardioversiei.
SOC MONOFAZIC SAU BIFAZIC ?
?
• Monofazic: sarcina electrica este administrate
intr-un singur sens de la polul negative spre
cel pozitiv, in timp ce in cazul socului bifazic
sarcina este transmisa de la negative la pozitiv
si ulterior de la pozitiv la negative.
• Socul bifazic este mult mai efficient dacat cel
monofaizc si sunt necesare energii mai joase.

COMPLICATII
• Aritmii nepericuloase: sunt cele mai frecvente complicatii: extrasystole
atriale, ventriculare sau jonctionale; nu trebuiesc tratate
• Fibrilatie ventriculara: poate sa apara daca se foloseste un soc asincron
care cade in perioada vulnerabila a undei T; in caz ca apare FiV se va folosi
un soc de energie maxima pentru restabilirea ritmului normal.
• Necroza miocardica: apare in general dupa orice cardioversie, dar este
nesemnificativa; in cazul socurilor multiple (ex: furtuna aritmica) de
energie maxima cresterea troponinei poate fi semnificativa, si persistenta
modificarilor de segment ST mai indelungata decat 1-2 minute
• Edemul pulmonar este o complicatie rara care apare fie datorita fractiei
de ejectie depreciate fie datorita “standstill-ului” atrial in cazul aritmiilor
atriale.
• Arsuri tegumentare dureroase.
