This document contains an acknowledgement form for students and parents/guardians to sign confirming they have read and understand the course syllabus and rules/expectations for an art class. It provides the teacher's contact information and requests signatures from the student and parent to confirm they have read and understand the class requirements. It also includes a section for parents to indicate if they have any skills, resources or collections that could enrich the art program.

any issues or concerns you may have. Students will be expected to include

reference throughout the semester please let me know.

In the art department we are always looking for parents, friends and
relatives who have jobs, skills, interests, hobbies, or collections that might
enrich our program. In addition, we are constantly looking for free sources
for supplies that can be used in our courses such as: paper, mat board,
plexiglass, masonite, fabric, etc.
