The active ingredient contained in Provanol
SR tablets is Propranolol Hydrochloride.
Each Provanol SR tablet contains 40 mg, 60
mg or 80 mg of Propranolol Hydrochloride.
7. Mechanism of Action

The mechanism of the antihypertensive effect
of Propranolol has not been established.
Among the factors that may be involved in
contributing to the anti-hypertensive action
include:
(1) decreased cardiac output,
(2) inhibition of renin release by the kidneys,
and
(3) diminution of tonic sympathetic nerve
outflow from vasomotor centers in the brain.

Provanol SR (Propranolol Hydrochloride
Tablets) is used to treat high blood pressure,
irregular heartbeats, shaking (tremors), and
other conditions.
It is used after a heart attack to improve the
chance of survival. It is also used to prevent
migraine headaches and chest pain (angina).
Lowering of high blood pressure reduces the
risk of fatal and nonfatal cardiovascular events,
primarily strokes and myocardial infarctions.
Preventing chest pain can help improve your
ability to exercise.

prevent migraines.
Propranolol can help reduce your
symptoms if you have too much thyroid
hormone in your body (Thyrotoxicosis).
You will normally take it together with
medicines to treat an overactive thyroid.

Propranolol Hydrochloride Tablets (Provanol
SR) must be used with caution in patients with
impaired hepatic or renal function.
Provanol SR is not indicated for the treatment of
hypertensive emergencies.
There have been reports of exacerbation of
angina and, in some cases, myocardial
infarction, following abrupt stopping of
Propranolol therapy.
Hypersensitivity reactions, including
anaphylactic / anaphylactoid reactions, have
been observed with the administration of
Propranolol tablets.

Cutaneous reactions, including Stevens-Johnson
Syndrome, toxic epidermal necrolysis, exfoliative dermatitis,
erythema ultiforme, and urticaria, have been observed with
use of Provanol SR tablets.
Sympathetic stimulation may be a vital component
supporting circulatory function in patients with congestive
heart failure, and its inhibition by beta blockade may
precipitate more severe failure.
Chronically administered beta-blocking therapy must not be
routinely stopped prior to major surgery, however the
impaired ability of the heart to respond to reflex adrenergic
stimuli may augment the risks of general anesthesia and
surgical procedures.
Beta-adrenergic blockade may prevent the appearance of
certain premonitory signs and symptoms (pulse rate and
pressure changes) of acute hypoglycemia, especially in
labile insulin-dependent diabetics.

Beta-adrenergic blockade can mask certain clinical signs of
hyperthyroidism. Therefore, abrupt stopping of Propranolol
tablets can be followed by an exacerbation of symptoms of
hyperthyroidism, including thyroid storm.
Beta-adrenergic blockade in patients with Wolf-Parkinson-
White Syndrome and tachycardia has been associated with
severe bradycardia requiring treatment with a pacemaker.
In the event of hemorrhage or shock, there is a
disadvantage in having both beta and alpha blockade since
the combination prevents the increase in heart rate and
peripheral vasoconstriction which is needed to maintain
blood pressure.

➢Hypertension: The usual initial adult dosage is Provanol
SR (Propranolol Hydrochloride Tablets) 40 mg twice daily,
whether used alone or added to a diuretic. Dosage can be
increased gradually until adequate blood pressure control is
achieved. The usual maintenance dosage is 120 mg to 240
mg per day.
➢Angina Pectoris: Total daily doses of 80 mg to 320 mg
Provanol SR, when administered orally, twice a day, three
times a day, or four times a day, have been shown to
increase exercise tolerance and to reduce ischemic changes
in the ECG. If treatment is to be discontinued, dosage has
to be gradually reduced over a period of several weeks.

➢Atrial Fibrillation:The recommended dose is 10 mg to 30 mg
Provanol SR (Propranolol Tablets) three or four times daily before
meals and at bedtime.
➢Myocardial Infarction: In the Beta-Blocker Heart Attack Trial
(BHAT), the initial dose of Propranolol Tablets was 40 mg three
times a day, with titration after 1 month to 60 mg to 80 mg t.i.d. as
tolerated. The recommended daily dose is 180 mg to 240 mg
Provanol SR per day in divided doses.
➢Migraine: The initial dose is 80 mg Provanol SR tablets daily in
divided doses. The usual effective dose range is 160 mg to 240 mg
per day. The dosage may be increased gradually to achieve
optimum migraine prophylaxis.
➢Essential Tremor: The initial dosage is Provanol SR (Propranolol
Tablets) 40 mg twice daily. Optimum reduction of essential tremor
is usually achieved with a dose of 120 mg per day.

Contraindications
Provanol SR is contraindicated in patients with a
hypersensitivity to Propranolol Hydrochloride or
any other component of this medication.
It is also contraindicated in cases of Cardiogenic
shock, Sinus bradycardia and greater than first
degree block and Bronchial asthma.

Do not stop taking this Provanol SR (Propranolol
sustained-release capsules) all of a sudden. If
you do, chest pain that is worse and in some
cases heart attack may occur. The risk may be
greater if you have certain types of heart disease.
To avoid side effects, you will want to slowly stop
this medicine (propranolol sustained-release
tablets) as ordered by your doctor. Call your
doctor right away if you have new or worse chest
pain or if other heart problems occur.

➢Propranolol Hydrochloride tablets are usually well tolerated
and side effects are transient in nature, rarely necessitating
withdrawal of treatment. The most serious Provanol SR
side effects observed are congestive heart failure and
bronchospasm in susceptible patients.
➢Common adverse reactions include fatigue and/or lassitude
(often transient), bradycardia, cold extremities and
exacerbation of Raynaud's phenomenon, sleep
disturbances including vivid dreams/nightmares.
➢Other less frequently reported side effects include:
gastrointestinal disturbances (anorexia, nausea, vomiting,
diarrhoea, abdominal pain), congestive heart failure,
dizziness, bronchospasm.
➢Rare cases of thrombocytopenia and purpura have also
been reported.
➢CNS symptoms including mood changes and hallucinations
have been rarely reported.

Most overdoses of Propranolol tablets
(Provanol SR) are mild and respond to
supportive care. Hypotension and
bradycardia have been observed following

appropriately.

Propranolol Hydrochloride ( Provanol SR
Tablets) is not significantly dialyzable.
Glucagon can exert potent inotropic and
chronotropic effects and could be particularly
useful for the treatment of hypotension or
depressed myocardial function after a Propranolol
overdose.
Serious bradycardia may require temporary
cardiac pacing.
The electrocardiogram, pulse, blood pressure,
neurobehavioral status and intake and output
balance must be monitored. Isoproterenol and
Aminophylline may be used for bronchospasm.
21. Propranolol During Pregnancy

Propranolol Hydrochloride ( Provanol SR tablets)
has been classified by the US FDA as Pregnancy
Category C.
There are no adequate and well-controlled
studies conducted in pregnant women.
Intrauterine growth retardation, small placentas,
and congenital abnormalities have been reported
in neonates whose mothers received Propranolol
tablets during pregnancy.
Neonates whose mothers received Provanol SR
tablets at parturition have exhibited bradycardia,
hypoglycemia, and/or respiratory depression.
