1. Three ways to associate your note
The third way to associate your note is to go to your portfolio of
notes and perform searches on each VC3 topic.
This can be done periodically.
The next 7 slides show how this is done.
2. Marking Notes as Examples of VC3 Problems
The first step is to find your home page on Portfolio
Next: Click on Learning Objectives
3. Summary of VC3 Problems in Clinical Notes
Here is a list of the 25
learning objectives.
Prior notes you have recorded (or recorded by the system
for you) are listed here.
Next click on finding matching
notes to see all the notes that
contain key abdominal pain
concepts.
4. KM Template for MCT Individual Meetings
These are notes that match for concepts of abdominal pain.
Click here to get more room to see the table.
5. KM Template for MCT Individual Meetings
Look through the table. You see your patient, the note,
whether it was recorded as a case of abdominal pain, the
concepts of abdominal pain found in the notes, the hits, the
score (google-type scoring system we created), and the
sections of the note in which the concepts are found.
If you confirm that this case was concerned with important
concepts of abdominal pain, you may click record and this
note will be added to list of notes recorded for abdominal
pain.
6. KM Template for MCT Individual Meetings
Scroll through these pages and record cases that are
associated with abdominal pain
7. KM Template for MCT Individual Meetings
Notes that you associate with abdominal pain appear on your
list of Events Recorded for abdominal pain.
Next search for matching notes for abnormal vaginal
bleeding. Record notes that associate with this topic. Then
do the same for the other VC3 topics.
8. KM Template for MCT Individual Meetings
As you record cases, your dashboard will be
updated on your home page.
9. Last question
Last question: should I associate all my notes I write?
Though not a hard-fast rule, for each patient to whom you are assigned
and asked to write notes, you should consider associating your H&P and
one progress note to VC3 topics that relate. If something changes in the
patient’s course, like developing a UTI (dysuria VC3 problem), then you’d
add that day’s progress note to dysuria on your dashboard if you discussed
the UTI in that note.
We ask that you do not associate notes to VC3 that are not written in
StarPanel (eg. VA notes, notes from clinics in the community). Such notes
require significant de-identification to be HIPPA compliant.
Shade Tree notes are fine because they are written in StarPanel.
