Discussed and worked on a project
incorporating forms on a website.
Worked with many form options including text
boxes, drop-down lists, option buttons etc.
3. OBJECTIVE
After a discussion on forms on the
Internet, students will be able to research
and identify forms on the Internet and how
they are used to collect information by writing
about their findings with 90% accuracy
through class discussion.
4. ACTIVITY INFO
Students had to find forms on the Internet.
Identified what kind of information was
collected in the form.
Identified what form options were used.
Example: option and radio buttons, text areas.
5. ACTIVITY
The students wrote a paragraph about each
website to identify:
Information that is provided on the site.
Information that is collected by the form.
Types of form options used.
6. ACTIVITY
After the students wrote about their 5
websites, we discussed as a class anything
interesting that they found about any form.
7. DIRECTIONS
Find 5 websites that incorporate forms.
In a paragraph for each, you need to do the
following:
Show the URL and title of the website.
Describe the purpose and information provided
on the website.
Describe the purpose and information collected
from the form.
8. RESULTS
Students really enjoyed activity.
Allowed them to identify forms that they use
everyday.
Did a great job explaining the purpose and
website.
