Vision Plans:
1. The Essential Vision Plan (Starting at 12.71/month for individuals)
➢In Network Providers Covered in Full
➢Out-of-network Providers cost between $36-$42
➢$125 allowance on single vision lenses
➢$120 allowance on contact lenses
➢Does not cover factory scratch coatings or poly-
carbonate lenses

➢In Network Providers Covered in Full
➢Out-of-network Providers cost between $36-$42
➢$150 allowance on single vision lenses

➢Covers poly-carbonate lenses, but not scratch
coating
