• At home I can manage my own health needs online and advise my
family and friends how they can too?
• At work I know how best to use existing digital systems in my job and
how they can improve the quality of our service and reduce any waste
• In my moments of inspiration or frustration I know where I can get
support to take my ideas forward and/or other organisations where
they’ve already made such improvements

Editor's Notes
The 10th July was my final day at NHS Digital, but I’m delivering today’s presentation promoting exactly the same message as I did when in post. Because we are all united by one common factor. We all want to live happy, healthy, stress free and fulfilled lives.
This photo was taken in 2011. It has history and a mass of underpinning hope and opportunity too. After introducing you to all the individuals in this picture I’d like you to tell me who is able, if they wish, to understand all their past diagnoses, view all their test results, add comments and updates to their records, see the value and costs of their medications to east their stress and improve their health?
I hope you will all be dreamers. But realistic, optimistic, pragmatic, energetic dreamers and, at the end of the day, dreamers who will be keen to take some practical steps to help others, and yourselves.
‘Boy in a coma’ – I was diagnosed with Type 1 Diabetes at the age of 6. My mam had shared with the GP information about my rapid weight reduction, constant thirst and sugary urine and yet no link was made, and the Doctor referred to things passing in conversation. Still along with insulin injections it introduced me to ‘a diet without tears’, getting blood tests and being given the results in hospital clinics
But now in the digital age are things any better?. How many of us have access to our GP records, or our full Medical Records? How many of us can share details of our own exercise regimes and test results? But one of the most important aspects to me is how many of us can support our elderly relatives and loved ones via encouraging them to ‘try digital’ and/or through having proxy access to records? The final picture is of my mother-in-law, Maureen whose experience perfectly demonstrates this.
From some workshop samples I’ve taken within the NHS:
Staff accessing their GP record is just above the nos in the national population – and yet it’s available to everyone! – We need to find out what would make it of value?
The majority have no access to information about their treatment and care in hospital
The majority collect information about their health, even though they can’t access information about their sickness, and are interested in proxy access to be able to care for family members
An even larger majority are interested in PHRs, - what would help them to understand and take action?

In terms of commitments following on from the great work that was done in particular by Amir Hannan with his patients such as the wonderful and now sadly departed Margaret Rickson, from 2005 onwards, “Think of Women’s Realm readers, Bruce!”. Self-management and self-care is part of national ambitions under the 5YFV and PHC 2020, and were at the forefront of Jeremy Hunt’s thoughts too. NHSX, the NHS App and every policy and initiative seem to support the importance of patient access and control. But except in isolated pockets, like in Amir, Yvonne and Richard’s area it just doesn’t happen on the ground. In fact you are made to feel like a member of the awkward squad.
Some personal reflections. We’re using ‘I’ statements throughout the strategy. But from a personal perspective we want people to be able to articulate what will make them feel ‘Digitally ready’ to pay their part in improving health and care across their own area. These statements are how I articulate it and I certainly am aware that I still have many unmet needs. But in addition to how articulate it to my friends and family you can find a full health and social care digital capabilities framework that was launched in 2018 on the HEE website. These questions are now what I judge every Health and Social Care Leader by because….
