1. ID CARD/DEERS INFORMATION
1) Recommend Sponsor obtain Power of Attorney (POA) from legal for spouse (General or Special DEERS
POA) regarding DEERS transactions.
2) If Power of Attorney (POA) is not available, Soldiers Deployment Orders can be utilized for ID/DEERS
transactions, but only during Wartime Contingencies.
3) Newborn enrollment (into DEERS) requires POA, Live Birth Record (this is a temporary Birth Certificate
from Medical facility where child was born that has parents and child’s information and requires
signature by nurse or physician), or State Birth Certificate along with Social Security Card (if one has
been issued to child).
4) After newborns are enrolled into DEERS, parent has to register child with Tri Care, located at Evans
Hospital, Ft. Carson CO.
5) New marriage enrollments (into DEERS) require valid POA, Marriage Certificate (Divorce Decree if
applicable), valid State or Federal Identification, and Social Security Card.
6) ID/DEERS facility requires “Lost Card Memorandum”, for all lost ID Cards, from local Provost
Marshall’s Office (Bldg #2700-Chrsitie St.). Please provide Power of Attorney at time of request. Note:
Adults are required to present two valid forms of ID at time of replacement, i.e. Drivers License & Social
Security Card.
