12. WHS Measures Week #17 Penalty Points Average Control Charts
Control chart rules
Rule 1 = Any point outside the control limits
Rule 2 = Seven consecutive points all above or all below the centre line, or all
increasing or decreasing:
Rule 3 = Unusual pattern or trends within the control limits:
Rule 4 = Number of points inside middle third of the region between the
control limits differs markedly from two -thirds of the total number of points:
Goal is to:
1. Identify special cause variation.
2. Investigate the causes
3. Implement corrective actions
4. Data driven actions leading to continuous improvements

13. Quality/Safety Alerts and Training Records
Important!!!
Training is delivered to all appropriate
staff and training records completed and
maintained for future audits
Alerts issued periodically where trends
or safety issues have been identified
20. Ventilation
When are Vents Required
All rooms that have had insulation installed in the walls must be ventilated in accordance with SR. 54 If no insulation been installed in a room then no
ventilation is required. An example of this can be occasionally found in Dormers houses. Bathrooms may be located upstairs not adjacent to gable walls. A
handy rule will be to ask yourself when you are in the room have I insulated any walls in this room if the answer is no, then no ventilation is required extract or
background ventilation.
Ventilation Requirements for Habitable Rooms/Kitchens/Utility Wet Room
If a habitable room/kitchen/utility wet room has had insulation installed then background ventilation is required. A new vent installed under the scheme must
achieve a free area of 6,500mm2 .
Ventilation Requirements for Rooms with Heat Producing Appliances
Where there is an open flued heat producing appliance present e.g. Open fire, stove, gas fireplace etc. a permanent vent must be recommended & installed for
this room.
What is a room sealed/open flued heat producing appliance? A Room sealed appliance is an appliance that takes air for combustion from the outside and
discharges the products of combustion (fumes) outside also, this is generally done by a balanced flue. These appliances do not require specific ventilation
related to the appliance. Background ventilation may still be required.
Part F requirements:
“Section 1.2.1.10 Ventilation systems should be designed by competent designers. Systems should be installed,
balanced and commissioned by competent installers e.g. Quality and Qualifications Ireland accredited or
Education Training Board or equivalent.”
·“Section 1.2.2.13 – “Where new mechanical extract ventilation systems are installed as part of a Major
Renovation as defined in Part L 2019, then the system should be designed, installed, commissioned and validated
as per 1.2.2.11.”
·Section 1.2.2.11 – “Systems when commissioned and balanced should then be validated to ensure that they
achieve the design flow rates by an independent competent person e.g. NSAI certified or equivalent………”.

21. •
MEVs terminating in attic space
MEV pipework not fully insulated
No MEV installed where required i.e. Kitchens,
Utility rooms & ensuites where wall insulation has
been installed
Mechanical Ventilation Top fails
22. Ventilation top fails
Background Ventilation Top fails
• Background vents not installed to standard
• Background vents not certified
• Background vents not fitted but required
• Vent covers not fitted to standard
• Blocked/obstructed vents
• Vents not closing
23. High efficiency gas top fails
• Permanent vent not installed where required (severity 1)
Ventilation Requirements for Rooms with Heat Producing Appliances
Where there is an open flued heat producing appliance present e.g. Open
fire, stove, gas fireplace etc. a permanent vent must be recommended &
installed for this room
24. High efficiency gas top fails
• Condensation pipework- Gas pipe not sleeved through
insulation and wall in accordance with Bord Gais Job Aid
Note: Condensate pipe work to drain, soak away, gulley or
appropriate terminal. • Note: all external condensate
pipework shall be run minimum 32mm diameter plastic
pipework. • Insulating 22mm pipework will not be accepted.
• Condensate pipework is to be insulated where running in
unheated areas
• Carbon monoxide alarm- Where any heating upgrade work
is carried out in a property under this scheme, a battery-
operated carbon monoxide alarm that complies with the EN
50291 standard must be installed. Where a flue passes
through a void a carbon monoxide as per applicable
regulations shall be installed
• Pipework not acceptable- Leak on condensation pipework
• Gas Supply- Not adequately supported
25. Roof insulation top fails
• Ventilation- No ventilation installed as recommended,
Vents blocked , Existing extract fans not ducted out,
just goes straight into the attic
• Pipework not acceptable- Not all pipes insulated
• Insulation depth installed
26. High-efficiency oil top fails
• Oil supply- In poor condition Gun Barrel fittings used on
oil line at tank,
• Condensation pipework- Condensate pipework in 22mm
not in larger diameter
• Carbon monoxide alarm- Not installed where required
• Pipework not acceptable- oil pipe leaking
