Program Coordinator
 Managed the La Camera and ensured that every clients needs were being met by
the staff. LaCamera is a high intensity group home that serves persons that are
with profound intellectual and physical disabilities .
 Supervised staff and provided support when needed.
 Informed my supervisor of any crisis and executed the appropriate measures
towards resolving the issues at hand.
 Completed weekly and monthly DDS and audit documentation.
Engagement Coordinator
 Completing adult comprehensive assessment of potential residents of the new
intensive group home that will be opening in April 2016.
 Coordinating clinical visits with collateral providers inpatient and other North
Suffolk Mental Health Association employees that will be working and managing
to new group home.
 Educating staff on appropriate interventions to utilize when engaging clients in
conversation about their transition to a group living environment from living
inpatient.
 Engaging clients in conversation along with their providers about the best way to
go about the transitional process to ensure that it is facilitated in a way that is the
least distressing for the client.
 Completing weekly clinical summaries from visits to update client’s treatment
team and collaterals on transition progress and any dilemmas or incidents that
arise.
Health Outreach Worker 7/14-1/16
 This is a contracted position funded by CMMI (Center for Medicaid and Medicare
Innovation) who have collaborated with Dartmouth College, Commonwealth Care
Alliance and Bosch Health System to develop a integrative care project that
focuses on decreasing hospital and ER visits through outreach services to adults
with co-morbid conditions, living in the community. The purpose of the pilot
study is to see if such services were available if it would make a major difference
in cost of care and quality of life for our persons served.
 Reviewing and educated persons served on material outlined in the Integrated
Illness Management and Recovery Model (IIMR) curriculum, developed by
Dartmouth College
 Follow up on Health Buddy (in home self-report computer units developed by
Bosch Heath Systems) alerts accordingly which would include sending a message
on the Health Buddy to client, calling or conducting a home visit depending on
the level of risk.
 Consult registered nurse on the team when pertaining to medical conditions.
Recreational Therapist 03/12-7/14
 Working alongside the clinical/leadership team, I developed an onsite day
program for the Greenway House, located at the Erich Lindemann Mental Health
Center.
 Creating lesson plans for structured groups that cater to the needs of the persons
served. The Greenway House serves as a transitional group home for those that
spent several years institutionalized and are now working on transitioning back
into the community and living more independently.
 Facilitating onsite groups that encompass but not limited to expressive therapies,
independent living/life skills, the recovery journey and wellness.
 Documenting billable IAP treatments/interventions offered in groups facilitated
by myself.
 Tracking the progress of each client and updating their treatment team.
 Coordinating both educational and recreational activities that help promote
personal independence, community integration and a better quality of life.
Assistant Program Coordinator 05/11-03/12
 Completed required paperwork, which included weekly medication audits, DMH
critical incident forms, CBFS progress notes, IAPs etc.
 Managed the Harborview House during the evening hours and ensured that every
clients needs were being met by the staff. Harborview is a high intensity group
home that serves persons that are medically and psychiatrically compromised.
This intensive group home consisted of a MIMI (Medically Ill Mentally Ill)
setting on the lower level.
 Supervised staff and provided support when needed.
 Informed my supervisor of any crisis and executed the appropriate measures
towards resolving the issues at hand.
 Created art-based activities for the clients and facilitated not only an art group for
my house specifically but also facilitated a bi-weekly art therapy based group for
the agency.
