Samantha Jordan Chanda will serve as an AmeriCorps VISTA member from August 21, 2015 to August 19, 2016 working with The Plantory in Kentucky on homelessness prevention. The goal of the project is to complete development and deployment of the OneDoorKY Coordinated Entry system for housing and homelessness in Lexington-Fayette, KY. From July 1, 2015 to January 1, 2016, Samantha will work to collaborate with providers and stakeholders to write and publish processes and procedures for the coordinated entry system and develop intake questions and forms to incorporate into the Homeless Management Information System, as well as train providers and stakeholders on the entry processes.
