This document provides instructions and templates for a healthy habit tracker to help users monitor their goals over multiple months. The tracker includes circles for each day of the month, with boxes at the top to write the goal being tracked such as drinking water, watering plants, walking to work, or reading before bed. Users are directed to select the corresponding circle for each completed day and fill it in with a color to represent achieving their goal for that day. The templates span three months to allow for long-term tracking.
