10. Clinical picture
A) Muscarinic stimulation:
1- miosis
2- Blurring of vision
3- cojuctvitis and lacrimation
4- nausea and vomiting and abdominal
cramps
5- salivation and sweating
6- bronchospasm
7- bradycardia and hypotension
11. B) Nicotinic Stimulation :
Muscle twitches ; weakness then paralysis
Respiratory muscle paralysis
Hypertension and bradycardia
C) Central effect :
Anxiety ; respiratory depression ; convulsion ;
collapse and death
12. Management of organ phosphors
poising
1- prevent further intoxication and get patient
away from contamination
2- remove contaminated clothes and wash skin
3- keep patent airway and aspirate bronchial
secretion
4- Gastric lavage
5- intubation and ventilatory support if needed
6- monitor bl. Pressure and heart rate.
14. 8- Oximes ( cholinesterase activator):
given as early as possible
:Toxogonin
Adult dose : 250mg iv (1 amp.) / repeated after 2hours
ped. Dose : 4 mg / kg
Avoid :
1- milk or oily demulcent
2 morphine
3- aminophilline

Inhibiting synthesis of vit. K dependant factor (2;7;9;10)
induction of anticoagulation leading to bleeding
Disruption of capillary memberane
22. Single acute ingestion:
no therapy required
Vit k (konakion) indicated in :
chronic and large ingestion
long acting compound
Doses :
adult : 5-25 mg im or sc . May be repeated once
mg im or sc1-5:children
Monitor PT to determine the amount of fresh frozen
plasma needed
