YEAR END ACCOMPLISHMENT REPORT
S.Y. 2014-2015
I – Introduction:
Teaching is noble profession. Thus, it calls for great responsibilities of a teacher to become competent,
well – equipped professional in teaching learners and dealing with all the school stakeholders. For these reasons,
teachers made an effort to participate in various trainings and other related activities in school and in the
community to meet those goals.
II – Specific target:
A.) Student Development:
1. Train students in the different contests during the literary and musical contest last November,
2014.
2. Encourage students to participate in the School Foundation Day Celebration.
3. Attend the students Christmas Party.
4. Conduct Readiness test to Math 9 and 4th yr. students.
5. Evaluate the outcomes of the students through giving quizzes, oral and written exams.
6. Attend the graduation ceremony of class 2015.
7. Celebrate Oral Reading Month last November , 2015.
8. Attend the mass during regular school schedules.
9. Conduct Home visitation to Students at Risk of Dropping out.
10. Conduct NAT Saturday Review
11. Raise the Mastery of the students by atleast 2 %
B.) Staff Development:
1. Conduct NCAE to Grade 9 in Cresencio S Lago NHS.
2. Prepare forms and laboratory and classroom room for evaluation –School level
3. Attend Inter-Agency Christmas Party at LGU, Marihatag, SDS.
C.) Curriculum Development:
1. Adopt and apply K-12 program for Grade-7 to Grade 9 subjects.
2. Apply 4 A’s in lesson planning for 4th year subjects
3. Initiate varied activities related to Mathematics Skills Development.
D.) Physical Facilities Development
1. Prepare Classroom room for structuring.
2. Support school ground beautification activities
3. Support the Gulayan sa Paaralan Program
E.)Management:
1. Assist teachers and students for Science laboratory hands on of materials and apparatus.
2. Keep the record of the Science apparatus, Computers, books and other reference materials.
3. Maintain the cleanliness and orderliness of the classroom and laboratory room.
4. Submit school reports on time.
5. Keep students scholastic records.

1. Attend Brgy. Fiesta and Town Fiesta celebration.
2. Participate the inter-Agency Family Day
3. Attend inter-agency Christmas party
4. Conduct Regular HRPTA Meeting and Attend GPTA meeting every quarter
G )Punctuality and attendance:
1. Come to school regularly.
2. Minimize absenteeism.
3. Attend school program, meetings and other community activities.
III – Accomplishment:

6. Attended Division Planning Conference of District and Cluster YESO And Science Coordinators
and SCAAP officers – Sept. 24,2014
