Objectives
I aim to gain employment in a social care role and contribute my experience, motivation and
enthusiasm to the sector. I have a particular interest in working with marginalised groups to overcome
barriers to community integration and employment.

Delivered a service to Support employees on the department of social welfares wage subsidy scheme.
This involved providing support to disabled employees regarding personal, social, health and family
issues which may prove as barriers to retaining employment. This included support in applying for
social welfare benefits and entitlements support re: money management. Enabling employees to
access community and health service support structures. Providing appropriate support, training and
guidance to both employees and employers. Liaising with supervisors/managers/HR representatives/
multi disciplinary team members relevant to the care and continued employment of the employee.
Provided disability awareness training across the companies and worked to support the managers in
various companies with issue pertaining to the employment of people with disability. Coordinated
with various agencies particularly the HSE and the department of social welfare. Managed a caseload
of 70 people with various disabilities and mental health providing for their support needs while in
employment. Aided the development of and maintained a comprehensive reporting system and

produced monthly reports in a timely manner. The monitoring of employee progress and changing

Provided services to people from various backgrounds receiving disability benefits and assisted them
to return to the workforce by providing social/emotional support, sourcing work and training
opportunities. Developed relationships with potential employers and supported them and the
employee on securing and maintaining employment. Provided necessary intervention to build work
skills, community integration and work capacity. Liaised with individuals, families and various service
providers to address barriers to employment. Ensured assessments were completed and adhered to

Assisted people with Multiple Sclerosis provided home support with daily living needs while also
working to provide social support through my role in the community outreach programme. Provided
day services and activities to interest/benefit a group of individuals with varied needs. Supported
clients with personal and medical care; bathing, grooming, dispensing and administering medication,
catheter and PEG feed care, feeding clients. Maintained written and verbal reports regarding client’s
daily activities and progress. Observed clients for medical issues/deterioration and reported findings
to relevant team members. Ensured that all required documentation was complete and accurate.
Reviewed and evaluated clients’ progress and made adjustments to programs as required. Provided
lay counseling/ emotional support and liaised with other agencies as required. Provided input to
management with regard to the development of the program and participated in program evaluations.

As a job coach my role involved supporting service-users with intellectual disabilities in maintaining
their jobs in a busy restaurant. In my care assistant role, I cared for individuals with severe/ profound
disabilities by meeting their support needs, assisting them with daily living and medical
requirements, reporting observations and liaising with other professionals involved. Managing
