Follows IDPH requirements
Conducted a needs assessment every 5 years
Stated their goal to “identify areas where we can have the largest impact on improving the quality of life for
all Kane residents- particularly the most vulnerable residents of your community- by reducing preventable
sickness and death” (CHIP 2011)
Also described the process and outcomes of their listed health priorities
They did this by using the National Prevention Strategy
With this they planned and assessed their Community Themes and Strengths and Public Health
Systems Priorities

CHIP had more data
DeKalb focused only on their 3 priorities, CHIP looked into other factors such as
socioeconomic status, access to healthcare, and their surrounding environments.
DeKalb had a simpler layout due to not enough data within

CHIP includes four priorites which address six top health threats and complement the goals of the
Kane county 2040 Plan.

- Reduce tobacco use and exposure to environmental tobacco smoke
- increase access to, and consumption, of fresh fruits and veggies
- Coordinate the effective communication of tailored, accurate and actionable health information
to Kane County residence across the lifespan.
- Create environments that prevents excessive consumption of alcohol

Priority #2: Increase Access to High Quality, Holistic and Preventive and Treatment Services
Across the Healthcare System
- Increase the proportion of residents of all ages that have regular, ongoing sources of medical
and dental care and residence who receive appropriate evidence-based clinical preventive
services
- Focus culturally appropriate outreach and engagement efforts to eliminate racial disparities in
health outcomes, especially in infant mortality.
- Enhance systems to support the prevention, early identification and treatment of communicable
diseases in the community
- Enhance systems to support the prevention, early identification and evidence- based treatment
of mental health
10. COMMUNITY HEALTH PLAN
Priority #3: Support & Create Health Promoting Neighborhoods, Towns & Cities
- Increase the availability and variety of high quality, safe and affordable housing compact, mixed
use development.
- Institute “complete streets” types of policies to ensure that roadways are designed and
operated with all users in mind including bicyclists, public transportation vehicle and riders, and
pedestrians of all ages and abilities.
- Assure access to safe playgrounds, parks, trails and open space.
- Assure access to safe food and clean and safe water and air.

● Priority #4: Promote Social, Economic and Educational Environments that Optimize
Health
- Increase the proportion of children who have high- quality early developmental
support, especially in care and education
- Increase the proportion of Kane County young people that complete high school
education.
- Increase the job skills and readiness of Kane County residents that are unemployed.

Accessibility:
Finding the Iplan was rather difficult at first as they refer to there Iplan as the Chip plan.
The Kane County Community health information plan is found as a pdf file on the health department's
main website page(www kanehealth.com)
Accessibility to other health assessments and information was easy to find and navigate on the
county’s website.
Data tables and charts and graphs throughout the document and in the appendix’s made accessing
information relevant to certain health topics easier and were easy to follow as well.
13. ACCESSIBILITY, ACCEPTABILITY,
AVAILABILITY
Availability:
If any individual in today’s society wants to know or search about health issues within Kane county
they can easily find readily available information online or by even calling or stopping into the county’s
health department.
Once on the County’s web page the information you are seeking can be easily found by navigating the
website and clicking there live links to current relevant items within the county.
On the county’s Chip document the table of contents helped make finding information quick and easy.
Tables and graphs helped finding data that was readily available and easy to analyze quick and were
easy to follow, clear and concise with information presented.
14. ACCESSIBILITY, ACCEPTABILITY,
AVAILABILITY
Acceptability:
The County’s IPlan Does meet and follow the IDPH’s requirements.
The county’s Iplan has an executive summary followed by the table of contents
found on the document.
Throughout the Iplan document all items seem to be in logical order. Easy to
read and follow and doesn't confuse the reader.
15. RELIABILITY, VALIDITY OF DATA SET
Reliability: refers to been dependability or consistency of a data set
Their data set is reliable in the sense that it’s in order, resourceful, updated,
and fully detailed.
Access to Health Care (Access to specialty care), Maternal and Child Health
(Infant mortality), Chronic Disease (Cardiovascular diseases), Access to Mental
Health Services, and Public Health Maintenance of Effort
16. RELIABILITY, VALIDITY OF DATA SET
Validity: Which means another word for “availability”. And it can be pointed out
that information may and may not exist and it if does, it’s not easily accessible.
Data set were basically followed and and in full details.
Implementing, making plans and working towards the outcome of the plan made
to fulfill priorities of improving community health.
According to Kane county IPLAN, they are required to conduct a mandated data
set every four years.
