Distribution
Centre
• Link worker to attend distribution centre meetups in
Knowsley to refer women on to the social prescribing
programme
1st Meeting -
Care
Merseyside
• Link worker arranges appointment to meet face to face with
woman. Baseline scores will be taken and a social
prescription is co-created and /or a referral is made to
Knowsley SP project via the platform
Follow up
meeting
• Link worker will contact the mum with via weekly phone calls
and will meet face to face after 6 weeks. Baseline will be
retaken. Activities will be added to the social prescription if
desired.

with the new mum and retake the scores and close the case.
If the woman still needs support she will be re-referred into
the social prescribing programme
