WHAT are you
noticing?
WHAT is helping or
hindering (or could in
the future)?
SO WHAT options have
you got to maximise
the helps and
minimise the hinders?
NOW WHAT action(s)
will you take?
1) What is the
population health
NEED we are trying
to meet?
2) To meet that
NEED what
OUTCOMES must
we achieve?
3) To achieve those
OUTCOMES what
QUALITY of
service /care must
we deliver?
4) To deliver that
QUALITY of
service/care what
CAPABILITY must
we develop?
Editor's Notes
Why triangles – back to the point about we like to think that cause and effect is linear and one way. Pull A and B changes but more usually B changing also impacts back on A. They are two way relationships and each factor is normally involved in more than one of those relationships at the same time.
Why triangles – it’s the simplest shape we could produce which showed more than two things being connected to each other . Pull on any one corner and it pulls the triangle out of shape.
Spaghetti Bolognese – another way of looking at it. You can pull it apart into its constituent parts, examine them eat them but that’s not Spaghetti Bolognese. The sum of the whole is greater than the sum of the parts. Everything is connected!
