Diet in Chronic Kidney disease non-dialysis patients
1. Diet in Chronic Kidney disease non-dialysis patients
Chronic Kidney (CKD) disease is common in the India and world wide. Diet plays an important role on the
progression of kidney disease. Over nutrition results in sodium and volume overload, hyperkalemia,
hyperphosphotemia and other toxins. Under nutrition leads to malnutrition and muscle wasting.
Diet in CKD depends on the stage on CKD, level of protein leaking, diabetes hypertension, or heart
failure.
There is no diet modification for patient with estimated glomerular filtration rate (eGFR) >= 60 ml/
min/1.73 m2
. For them the diet is same as general population.
Diet For patient with eGFR < 60ml per ml/min/1.73 m2
and not on dialysis:
A daily protein intake of 0.8 gram per kg. Very low protein intake is not recommended.
For non-nephrotic patients we suggest daily protein intake should be < 0.8 g per kg per day.
Indian patients are usually stop all forms of protein diet which is wrong. Specially those are vegetarian
they stop all forms of pulses and dairy products. They should continue taking some amount of protein
and take pulses and dairy products. Non-vegetarian can take egg white and limited amount of non-
vegetarian food.
For Nephrotic syndrome protein intake should be 0 .8 g per kg per day + 1 g of protein for each gram of
protein leaking.
Source of protein intake.
A diet rich in vegetable is recommended.
Sodium intake depend upon blood pressure, volume overload, and protein leaking. Sodium intake
should be > 2 g/ day (5 g/day of salt).
The potassium intake should be restricted. Low potassium in diet. CKD stage 3-4 the potassium intake
should be 2 to 4 gram per day(51 to 102 mEq per day) .
Maximum phosphorus intake of 1 g per day. For
Maximum Calorie intake of 32-35 K Cal/kg a per day.
Maximum fat intake <39 percent of daily Calorie intake.
