1. Challenge
To further its mission, the Department,
in collaboration with the Partners in
Care Foundation and the St. Barnabas
Senior Center, sought a creative new
way to enhance the physical, mental,
and emotional health of multi-ethnic,
low-income seniors living in Los Angeles
through the Senior Health and Wellness
Connection project.
Solution
Get Real Health joined forces
with Microsoft to help the partner
organizations develop a multi-faceted
Community Center-based program that
combined opportunity for increased
physical activity, socialization, self-
monitoring of chronic conditions, and
health education.
We developed a HealthVault-integrated
customized InstantPHRTM
solution for
the centers to help them facilitate
screenings, conduct health surveys and
assist seniors in monitoring their blood
pressure, blood-glucose levels and other
health metrics. Seniors in the program
were also given the opportunity to use
Kinect for Xbox 360 to make exercise fun
and to supplement other fitness activities
such as tai chi. Using the personal health
record tool, the seniors and their wellness
center caregivers are able to track their
activity levels and key health metrics,
as well as review and share their
information with medical professionals
and family members, all within one
easy-to-use interface.
