South London Maudsley NHS Trust
Foundation sought to develop an
interactive patient portal to support
their commitment to a new, connected
health model. They were piloting an
“eMPOWERMENT Program” that would
empower patients to hold a larger stake
in their mental health and well-being as
well as foster a sharing of data between
different care organizations facilitated by
the patients themselves.
Solution
Get Real Health developed
“myhealthlocker” built on the
InstantPHRTM
platform which works with
Microsoft HealhVault and allows SLaM’s
service users to gather, store and share
their own health information.
Through the myhealthlocker service,
users connect with their data from SLaM
and their local general practitioner as
well as providing the opportunity to
provide feedback through patient rated
outcomes straight to SLaM’s electronic
medical system. The solution includes a
completely customizable health journal
allowing patients to track sleep, mood
and behavior patterns over time utilizing
sliders, icons and many other formats.

in mental health. It is pioneering a
connected health model that for the first
time, links primary care, secondary care,
researchers and service users in a holistic
information ecosystem; whose function
is to empower and enrich the user’s
health experience.
