Challenge
The ADA sought a consumer friendly
diabetes centric personal health record
that allowed patients and consumers
the ability to track their key health
information, share information with
family members and access information
associated with their health.
Additionally, ADA wanted to make
this personal health record relevant to
multiple stakeholders of their association
from those consumers who were healthy
but seekers of health information to
those patients managing the complex
condition of Diabetes.
Solution
In this instance, InstantPHR™ was
skinned and branded to the look and
feel of the ADA and configured using
the InstantPHR™ toolset of widgets.
This application also stores patient data
directly into Microsoft HealthVault with
InstantPHR™ as the data visualization
layer for ADA.
This application is hosted by Get Real
Health and available through the ADA
website for the millions of consumers
who visit the ADA website each week.
In Diabetes 24/7, there are many
information sets that are collected.
Additionally, the system integrates with
over 75 home health care devices and
these values are visualized in graphical
and tabular formats within the site. Home
devices most widely used include blood
glucose meters, blood pressure monitors,
weight scales and pedometers.
Benefits
This solution allows not only people with
diabetes, but also family members one
central resource for personal health data
as well as reference information to assist
in coping with this chronic disease.
Because it is skinned to ADA guidelines
and specifications, there is a continuity
in branding that users will recognize
and trust.
