8. REMINDERS:
● All graduates shall be reported or included in the quarter where in he or she
was enrolled. (i.e. Client A is enrolled in Quarter 1 of CY 2022. After
completion of the treatment/program Client A will be included in the list of
graduates of Q1 CY 2023).
● Reporting of number of graduates shall not be accumulated
● Submission of all CBDRP reporting form for CY 2023 must be submitted on or
before January 30, 2024.
