As part of my research thesis at the University of Southern Mindanao, I
am conducting a survey that can answer about Dance Effects on Mental, Emotional
health and self -confidence among Junior and Senior High Dancer: A Comparative
Study. I will appreciate if you could complete the following table. Any
information obtained in connection with this study that can be identified with
you will remain confidential.

Do you think you could recall the combinations learned from today’s classes?

Do you feel better, mentally after dancing?
