2001 – 2006 Maricopa Ear, Nose & Throat, LLC (Tempe, AZ) Dr. Thad Bartell, M.D. & Dr. Douglas
Smith Back office supervisor for two ENT surgeons, responsible for hiring and training of back office
staff, patient education, surgery scheduling, coordination of benefits and in-office surgical assist.
1999 - 2001 Thomas Davis Medical Group (Tempe, AZ) Dr. Allan Wachter, M.D. & Dr. Phillip
Husband Back office assist for both allergy/immunology clinic as well as pulmonary office.
Performed multiple types of allergy tests, prepared serums, assisted on thoracentesis, PFTs and
operated allergy clinics.
1996 – 1999 Casa Blanca Medical Center (Gilbert, AZ) Dr. Dean Faust, D.O. Primary Care Physician
Nurse for PCP in large clinic. Primary assist to numerous procedures including gynecological measures,
in office biopsies, casting/splinting, pain management, patient education and care coordination via
insurance carriers.
1993 – 1996 St. Anthony Home (Crown Point, IN) Long Term Care Facility
Head nurse for intermediate and locked Alzheimer’s unit. Provided direct patient care services
including but not limited to hospice services, wound care, tube feedings, and medication management.
Communicated with family and facility ‘house’ physician regarding patient status and care
coordination.
1991 – 1993 Whispering Pines Health Care (Valparaiso, IN) Long Term Care Facility
While attending classes during the day, worked as a Certified Nursing Assistant directly caring for
skilled patients requiring bathing, feeding, and extensive assist with activities of daily living.
I hope this gives you a better view of me and that my experience conveys my general care and
concern for the overall quality of healthcare. Communication to patients/members is
something that I have advocated for since the very beginning of my career. Regardless of
barriers, i.e., socioeconomic, language, knowledge deficits etc., I wholeheartedly want to be an
integral part of an organization that clearly keeps the focus on the members/patients that are
the heart of our health care system.
