In the Description that is being requested to
create or change, describe what you want to
happen (change) in this box to your form or
procedure
In the description of what the request will be
changed to.
Who is making the request
The date of the request was submitted- type
in or use the drop down calendar
The date of the request is needed. Type in or
used drop down calendar
When every thing is completed, click on SUBMIT
Any supporting documents that is needed to make
changes. Scan in and attach to attachment.
Attachments can be examples, drawings, marked
up documents and any documents that support the
request. This will also be a place to store any edits
made to the requested document to show reasons
why these changes occurred.

Everything that was entered in is populated into the list. This
information an be used to research back what, when, why a
particular document was created, revised or obsoleted with all
the attach documents to serve as history for that particular
record. This is the purpose for the DCR number.

Then once your request has been filled out, just send the person
making the changes an email stating that you have sent in a new
document request requiring work. The person can then go to the DCR
request portal call it up and view the request. When it is complete the
editor will let you know through email that it is completed and email
you the updated form, file, document, procedure for your review and
if everything is sufficient let the editor know so the submitted
document, procedure, form can be sent for approvals.

This form can be found on
SharePoint at Quality>Quality
Documents>Admin>Document
Change Request>DCR Form
Fill all information as indicated
and submit to the Quality.
