To Err Is Human Analysis
After reading brief, To Err is Human: Building a Safer Health System, I did a little research and happened to find an article from April 2015 that
talked about To Err is Human. This article discussed several topics but one that stood out was that a few analysis claim that To Err is Human
counted too few errors. Yes, too few. The studies performed for To Err is Human were based on what was written on medical errors. Some doctors
often leave off mistakes in medical records, which would have lead to a lesser amount of errors reported. Towards the end of the article it discusses
how hard it is to know on a broad scale whether things are getting better, which goes along with the issue as to whether or not conditions have

Plagued with addiction, health issues, emotional distress, and a rough upbringing, Tennessee Williams was able to put these ailments to the side and
carry a career as an accomplished author, poet, and playwright ("Biography" 1–2). In Tennessee's novella The Glass Menagerie, a story of a family's
struggles taking place in St Louis, Missouri, 1937, much of the story's plot and characters have a strikingly similar reality to that of Tennessee and his
own personal life. In particular, one of the main characters and the story's narrator Tom Wingfield shows a life with close correlation to that of
Tennessee's. It can be concluded that Tennessee Williamsuses his art and talent for writing as an outlet to express his emotional pain. Williams
shows that emotional pain can be expressed through the process of art by the tension Tom's mother creates, the love for reading and writing, and the
need for adventure. Williams shows that emotional pain can be expressed through art by showing a connection between Tom and his own mother's
inability to accept their sister's potential and proficiency in life. In the case of Tennessee and his sister, Tennessee's mother Edwina wasn't truly
accepting of his sister Rose and was convinced to have an operation on Rose's brain to fix what was seen as "erratic behavior" ("Biography" 1).

Julius Caesar, the man who turned Rome into a force to be reckoned with was a great leader, however his long and powerful rule was ended with a
simple assassination because he abused his power after dictating the stronghold of Rome. History has proven to the Historical community that if one
obtains power they are likely to misuse that power. The two authors that will be used in my literary analysis essay are Coldplay and Alexandre Dumas.
The two text that will be discussed are The Count of Monte Cristo and Viva La Vida. Each paragraph will be relating diction used by both authors to
prove a point. In the Count of Monte Cristo Edmond Dantes a sailor is framed and put in jail. After many painful years he escapes to put forward
his revenge plan. Viva La vida is a hit song/poem; the lyrics imply that a man once was a king, and after being used he is overthrown, and a new

of Monte cristo are two sad and dark stories. In Viva La Vida Chris Martin writes "Just a puppet on a lonely string," (Chris Martin 31). In the Count
of Monte Cristo Alexandre Dumas writes " Monte Cristo cast a rapid glance around him. Andrea had disappeared," (Dumas 56). The word choice in
Viva La Vida is extremely significant because Chris Martin uses the words "puppet on a lonely string." Puppet must be a metaphor to the fact that he

To err is human. Throughout everyday life, human error is around every corner. Human error is defined as, "a mistake made by a person rather than
being caused by a poorly designed process or the malfunctioning of a machine such as a computer." (Encarta, 2009) To simplify this definition, people

Shift work can require some employees to work when the majority of people are resting. In some cases, shift work requires rotating between two or

While many companies are turning to shift work there seems to be an alarming trend in the influence of night shift and accident rates. Research has

Although there is some debate about when accidents most frequently occur, there is ample evidence linking non–standard hours of work to serious

Forgiveness Reflection
Forgiveness is a concept that most people do not understand, and I must admit I was one of those people before I took the class. My experience with
this concept was mainly through church sermons where the pastor explained God forgave us of our sins to restore a relationship with us. Therefore,
my definition was clouded by these sermons and I would have said forgiveness was saying you are sorry no matter who was at fault, showing remorse
for your actions, changing your actions, and reconciling to the point of being civil to the person that had wronged you. Thankfully, because of the
class Forgiveness & Communication, I have obtained information that will empower me to move forward toward a more forgiving life style.
Through class lecture, class participation, and different types of resources I have learned many things including, the need to forgive, what
forgiveness is not, what forgiveness is, effective methods of letting go, and the tools to know if you have truly forgiven and are now different. The
need to forgive affects us physically, mentally, relationally, and spiritually. The intensity and duration of the unforgiveness can cause it to become
debilitative. Accordingly, unforgiveness does not affect the other person, it affects you negatively. For instance, like the quote of Marianne
Williamson "unforgiveness is like drinking poison yourself and waiting for the other person to die." Research shows a lot of our sicknesses such
as high blood pressure and heart trouble are intensified by the lack of our not letting go of past hurts. Mentally we spend time thinking about a past
transgression more than we spend time thinking about happy events in our lives. I think it is like ivy growing in your mind, you first think the ivy is
pretty but then it starts to grow and eventually covers the wall of your mind and chokes out all other flowering vegetation, leaving you with only the
transgression and no beautiful memories. If we do not forgive it will affect our relationships and can eventually cause the relationship to cease to be.
Lastly, our spiritual growth can be hampered by our lack of forgiving and we cannot have peace in our lives. Booker T Washington once said, "the

As a pilot I get sick of hearing the term "human error" whenever an accident investigation releases their report. Almost every possible scenario can
get traced back to human error in one way or another: pilot failed to initiate proper emergency procedures, pilot failed to stop when he should have
stopped, pilot failed to go when he should have gone, and pilot was looking left when he should have been looking right. It is almost too easy to

The aircraft originated from Miami Florida and was just 33 miles northeast of its destination when it crashed into mountainous terrain killing all but 4
passengers. The accident investigation determined that there was no indication of any aircraft systems malfunction, no indication of any unusual
meteorological event, no indication of any external hostile force acting on the aircraft, and no indication of any out–of–service condition of any
applicable ground based navigational aids (Ladkin). So what was the cause of this accident? Apparently, when the pilot intended to proceed direct to
the navaid "ROZO" he entered an "R" into the flight management system (FMS) which defaulted to a navaid over 100 miles away called "ROMEO."
The autopilot automatically started steering toward "ROMEO" while the pilot continued his descent for what he thought was the final approach.
Unfortunately for everyone onboard, mountains existed in the direction of "ROMEO" and 160 people died (Anderson). Had the pilot not made the
error of entering an "R" instead of "ROZO" this accident would have been prevented....what a difference 3 letters can make.
While others are quick to look at this accident and yell "pilot error," I wonder why they aren't yelling "autopilot error" or "FMS error." I couldn't
even begin to tell you what an FMS looks like, much less how to use one, but I can tell you that upon closer examination of this accident we may want

Another personal favorite scripture is also written in the gospel of John, "For God so loved the world that he gave his only Son, so that everyone who
believes in him might not perish but might have eternal life (3:16). John is emphasizing the relationship between Father [God] and Son [Jesus].
He is saying that the Son has come from the Father and in order to see the Father one must first believe in the Son (125). Divinity is the very
character of God. Therefore, Jesus' divinity is made known repeatedly throughout the New Testament. In the very first words of the Gospel of Mark
1:1, Mark writes "The beginning of the good news of Jesus Christ, the Son of God." It establishes him as the manifestation of God in human form.

Throughout history many people believed that Jesus Christ is simultaneously human and divine because he came onto this earth 100% human and
100% divine. Jesus is both human and divine because of this evidence. The first evidence is Appearance, the second evidence is Miracles, the third
evidence is Human Qualities and the last evidence is Birth, Death and Resurrection. The first evidence is the way Jesus dressed and apperaed to people

The discussion regarding the humanity and divinity of Jesus Christ has been a matter of debate since the earliest days of Christianity. The
post–apostolic church became plagued by different heresies, some of which continue to this day. Generations of scholars have attempted to make sense
of the human and divine natures coexisting in Jesus, some claiming that He could not be truly a man, others claiming He could not truly be God;
however, the Bible tells us that the people closest to Jesus, the apostles, were convinced that he was truly God and truly man; their evidence was what

Early heresies arose as some scholars attempted to answer these questions, some saying that Jesus wasn't fully God, and others stating He wasn't fully
man. For example, Apollinaris (ca. 300–315) believed that Jesus was indeed a man, but instead of possessing a soul, as we do, He was possessed by
logos. Arius (ca. 250) felt that since God was immutable, there could be no way that the substance of God could be communicated or shared with
another being; therefore, Christ was a created being and was not pre–existent. These ideas were dangerous in either overemphasizing or denying the
God–man's attributes. Jesus had to be both God and man; otherwise, His sacrifice would have been insufficient or null. If He were a man with logos
instead of a soul, as Apollinaris suggested, Jesus' death would not have saved our souls. If He were not truly God, made flesh, then His sacrifice
would not have saved our flesh. Jesus, therefore, had to be both, to satisfy the demands of a perfect sacrifice and have the power to judge and forgive
sins. The ideas that Jesus was anything less than a perfectly joined union of God and man were countered by the Fourth Ecumenical Council in
Chalcedon, located in what is now Turkey, in A.D. 451. This council was convened with the purpose of defining the hypostatic union of Jesus' humanity

Happiness consists in reason and there is a highest good which does not come to us by chance but can be achieved by connecting a rational soul with
virtue. Although happiness is not virtue it is virtuous activity. According to Aristotle, virtue is among the traits which contribute to creating and

Throughout the course of humanity's history many questions have risen that have posed a problem to us as a species. Questions such as the following:
What is the proper relationship between individuals and society? What is beauty, and why is it important? Is there purpose in human existence?, and
many others have caused enough trouble in humanity's short existence on the earth to cause a near identity crisis. However, all of these questions fall
under the shadow of the main question that has been asked since the creation of Adam– "What exactly is the real relationship and purpose of humanity
towards the divine?"
During the time of philosophers, such as Aristotle and Plato, "What exactly is the real relationship and purpose of humanity towards the divine?" was
the question spreading among the philosophers. When questioned what God is, Aristotle stated in writing, "God is a supreme and eternal living
being, so that to God belong life and continuous and eternal duration. For that is what God is." If we choose to believe Aristotle's logic then we are
left without a reason as to why humanity exists and are left to make assumptions about our existence for the divine power. If God is eternal and has

Although many of the available studies have focused on the hospital setting, medical errors present a problem in any setting, not just hospitals.
Errors are also costly in terms of opportunity costs. Dollars spent on having to repeat diagnostic tests or counteract adverse drug events are dollars
unavailable for other purposes. Purchasers and patients pay for errors when insurance costs and copayments are inflated by services that would
not have been necessary had proper care been provided. It is impossible for the nation to achieve the greatest value possible from the billions of
dollars spent on medical care if the care contains errors. But not all the costs can be directly measured. Errors are also costly in terms of loss of trust
in the system by patients and diminished satisfaction by both patients and health professionals. Patients who experience a longer hospital stay or
disability as a result of errors pay with physical and psychological discomfort. Health care professionals pay with loss of morale and frustration at not

My experience with this concept was mainly through church sermons where the pastor explained God forgave us of our sins to restore a relationship with us.
