The Application of ISO 9001 Quality Management System in My Life

My medical check up (6.3 and 8.5.3) was stunned by the diagnosis with an
End Stage Renal Failure (8.3). The medical doctors reviewed the options of
Renal Replacement Therapy (7.2.1), such as Haemodialysis and Continuous
Ambulatory Peritoneal Dialysis (CAPD), and its limitations.
After careful selection and evaluation (7.4.1) of the characteristics of each of
the options, in meeting my working environment (5.2), I opted for CAPD of
Baxter Health Care.
I religiously carried out the four bag exchanges daily (7.1), with infusion of
two (2) litres of dialysate into my abdomen through a surgically inserted
Tenchkoff Catheter. The dialysate came in three different concentrations, i.e.
1.5%, 2.5% and 4.25 % of dextrose solution. Each bag exchange would last
for 30 minutes duration.
Before the conduct of each bag exchange, I wiped the table in the dialysis
room clean to ensure it was free of dust (6.4), before placing the dialysate
solution and consumable on top. After which, I wore a face mask, and
scrubbed my hands for five (5) minutes, wiped dry, and sterile before
carrying out the bag exchange as per CAPD manual (4.2.3 (f))
At the end of the bag exchange, I went about my daily activities, only to
return to repeat the same after a threshold of 4-5 hours. I would check the
discharge to ensure it was free of any blood stains or cloudiness (8.2.3 and
8.2.4) before disposing into the sewage.
Once every fortnightly, I would go back to hospital to collect my supplies of
consumables and store them in my dialysis room. The dialysate solutions
that came in bags were stored and rotated on a First in First out (FIFO)
inventory basis, based on the date of manufacture or expiry. (7.5.3 and 7.5.5)
All of the daily monitoring of bodily weight, measured blood pressure by
Sphygmomanometer (7.6) that were appropriately recorded and kept (4.2.3

and 4.2.4), were dutifully submitted to the Nephrologists or Nurses of Kuala
Lumpur Hospital for evaluation my Glomeruli Filtration Rate (GTR) (8.4).
Once every three (3) months, I followed up in the hospital where doctor
would exam (8.2.2.), review (5.6.1) and analyze results of my blood tests;
prescribe my medication (7.3) based on the blood results, to ensure I
continue to remain well and healthy (8.5.1)
I successfully underwent a deceased organ donor transplant of a kidney
(8.5.2) on 27 July 1996, and had been doing well since constant walking and
wai-tan-kung or tai-chi exercise.
I would like to record (4.2.4) my ever gratefulness to the family of my
deceased organ donor, Mr. CH Bender, who had altruistically in their most
difficult time, donated a cadaver kidney, the Gift of Life. I resolve take good
care my grafted kidney (7.5.4) through regular medical follow up in Kuala
Lumpur Hospital, watchful of my diet with more of fibrous food and lots of
fruits, and religiously take my immunosuppressive medication twice daily
(8.2.4).
Regularly, I update myself with the latest development in kidney transplant
(6.2.2) through reading reference articles from the webpage of Critical
Research Centre (6.3) to be competent, in providing counseling to newly
diagnosed patients or transplanted recipients (7.2.3)
I am committed (5.1) to lead a simple and healthy life (5.3) and to go for
holidays once yearly (5.4.1).
My wife, who is my Management Representative (5.5.2), will discuss (5.5.3)
in order to prepare a suitable diet (8.2.1) for me.
