Blood Transfusion Protocol
1. ❖ Identify Your Patient
Name, Relation with Employee, Age, Sex, Book No, Post
❖ Confirm Dr/Specialist’s Prescription (Written Order)
❖ Confirm Blood Type & Volume
Whole Blood, Packed Red Blood Cells, Platelets, FFP
❖ Take Informed Consent
❖ Take Vital Signs (BP, TPR) Of Patient
Before, then after 15min, after half hr. then Once in every hr. for next 3 hrs.
❖ Check Labels on Blood Bag
❖ Thorough Check of Blood Bag for Hemolysis
❖ Blood Group & Rh Factor (Donor & Patient)
❖ Donor Name
❖ Cross Match / ABO Compatibility
❖ Blood Bag Number / Blood Bank Record Number
❖ Screening For HBV, HCV, HIV
❖ Blood Collection Date
❖ Expiry Date
❖ Validate / Recheck by Another RN Or DMO
❖ If Above Everything is OK then Pass Fresh I/V Cannula of 18
OR 20G Start Blood Transfusion
❖ Closely Observe the Patient in Initial 15 Minutes Because
Severe Blood Transfusion Reactions Are More Likely to
Develop in First 15 Minutes
❖ Document the procedure including all information
❖ Observe pt. for acute <24hr & Delayed >24hr Reactions,
Including Transfusion associated circulatory overload (TACO)
General Knowledge
❖ Stored blood less than 7 days old is termed “Fresh Blood”
❖ Determine patient’s allergies & previous blood transfusion reaction
❖ Administer blood within 30 minutes of receiving from Blood Bank
❖ There is no evidence that warming blood is beneficial to the patient
when transfusion is slow.
❖ At transfusion rates >100 mL/m, cold blood may be a contributing
factor in cardiac arrest
Blood should only be warmed in a Blood Warmer. Blood warmers should
have a visible thermometer and an audible warning alarm
Warmed blood is most commonly required in:
1) Large volume rapid transfusions. 2) Adults: more than 50 mL/kg/hour.
2) Children: more than 15 mL/kg/hour. 4) Exchange transfusion in infants.

Blood Donor Recruitment
Appearance; Good physical & Mental health.
Age: Donor shall be between 18 and 60 years of age
Hb: shall not be less than 12.5 g/dL for males and
11.5 g/dL for females.
Weight: Minimum 45 kg
Blood pressure: shall be normal (systolic: 100‐140
mm Hg and diastolic: 60‐90 mm Hg is recommended),
without the aid of anti‐hypertensive medication.
Temperature: Temperature shall be Normal
Pulse: Pulse shall be Normal and Regular.
Donation interval: shall be 4 months
